Public Health Nurse II

The City of Alexandria is located in northern Virginia and is bordered by the District of Columbia (Potomac River), Arlington and Fairfax counties. An Overview

The Public Health Nurse II supports and empowers older adults and caregivers to achieve and maintain an optimal level of health in the community through home visits and care coordination with service providers. Services provided by the nurse are designed specifically to prevent unnecessary emergency room visits, hospitalizations, and premature placement in long-term care residences. The Public Health Nurse II will join a multi-disciplinary team of dedicated professionals including Family Service Specialists/social workers, mental health therapists, and other aging specialists. The Public Health Nurse II will be responsible for providing direct nursing care and case management for an assigned caseload, to include work on state mandated pre-screenings. Home visits constitute approximately 80% of working hours and entail providing initial and comprehensive assessment of clients’ medical and psychosocial needs.

What You Should Bring

The ideal candidate Knowledge of the principles and practices of public health nursing and/or case management and of measures and techniques used in the prevention of communicable diseases. Knowledge of, theories of human behavior and group behavior and normal and abnormal development. Knowledge of aging, health, intellectual disabilities, mental illness, trauma and other problems and processes that affect development. Knowledge and skill in medication administration including: actions, expected results, contraindications, and side effects. Knowledge of social problems that affect functioning such as current social, economic and health problems including poverty, AIDS, homelessness, drug addiction, and nutritional problems and the dynamics surrounding these issues. Good knowledge of therapeutic treatment techniques, good assessment skills, interviewing skills, and crisis management skills.

Ability to work independently; interpret laws, policies and regulations concerning human services; develop appropriate permanency plans; and ability to apply casework and group philosophy, methods and practices. Ability to work successfully with clients and to aid them in the constructive use of their own capabilities; ability to train and lead others; ability to communicate clearly and effectively both verbally and in writing.

The Opportunity
  • Conducts comprehensive clinical assessments in the homes of Adult Protective Services, Adult Services and/or Older Adult Mental Health clients with severely compromised health status, providing initial and comprehensive assessment of clients' medical and psychosocial needs. This includes completing that part of the Long-Term Supports and Services pre-screening assessment that does not need to be completed by staff in the Health Department;
  • Provides ongoing medical monitoring and education of clients and their caregivers, including monitoring of multiple medications received by many clients and follows-up as needed. Sets up weekly medications for those clients who can self-administer after the medications have been set up; administers medications as needed;
  • Formulates care plans, documents observations, care provided and effects of care on patient in accordance with established recording requirements;
  • Provides the medical screenings for homebound clients that are required to facilitate placements in nursing homes or assisted living, (i.e. TB screenings) or help with specific protocols.
  • Consults with physicians, other health care providers and community agencies to explore resources for care and ensure continuity of service;
  • Serves as a resource to colleagues in the Division to provide guidance, advice, and to assist with setting up safe health education and compliance strategies with treatment recommendations for clients and their families;
  • Provides information and referral services via phone, e-mail or in person about services available to older adults and adults with disabilities one day per week.
  • Prepares a variety of required/requested reports.
  • Perform any related duties as assigned.

The Center for Adult Services (CAS) provides programs and services in Aging; Adult Protective Services; Clinical & Emergency Services; and Community & Residential Support Services for seniors and for persons with mental illness, intellectual disabilities and substance use disorders. CAS provides compassionate and effective person-centered services based on best practices. Our teams support self-determination, safety and recovery for Alexandria residents affected by abuse, neglect, mental illness, intellectual disabilities and substance use disorders.

Minimum & Additional Requirements

Four-Year College Degree from a school of nursing of an accredited college or university approved for the inclusion of public health nursing; three years of responsible public health nursing experience that included one year of experience providing services to individuals and families in a variety of settings such as community, clinics and schools; or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. Eligibility for, or current licensure with the Commonwealth of Virginia as a registered nurse, and such other qualifications which may be prescribed by the Virginia Department of Health or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. Must possess a valid driver's license.

Preferred Qualifications

At least one year of experience providing community based chronic disease services, which includes making home visits. Fluent and multi-lingual in Spanish, Amharic, Farsi and/or Korean. At least one year of experience working within a multi-disciplinary team; At least one year of experience working with adults with behavioral health issues; At least one year of experience in providing nursing case management in the community; At least one year of experience working with multi-cultural populations; ability to communicate with people with limited-English-speaking ability.

This position requires the successful completion of pre-employment background checks including but not limited to: FBI/Federal Records Check; VA State Child Abuse/Neglect Registry; Medicare/Medicaid Fraud Database; Verification of education/certifications; Drug screening.

Schedule:

Monday through Friday, 8:00AM - 5:00PM. There will be times that the incumbent will need to be available in the evening.
