Position Description:Case ManagerFriends of Guest House, located in Alexandria, Virginia, helps women successfully reenter the community from incarceration. Since our founding in 1974, Guest House has helped more than 3,000 women break the vicious cycle of incarceration, reunite with their families, and reintegrate into their communities.Position Summary The Case Manager works as part of the client management team to facilitate all client engagement and case file maintenance, along with individualized treatment planning and implementation.Responsibilities & Duties:Case management Conduct program orientation for new residents within first three daysConduct intake meetings with assigned residents and develop an Individual Service and Goal Plan for each new resident within 7 business days of their entryProvide one-on-one case management services to residents, including advocacy, information and referral services (eg, employment, housing, social services) (minimum case load of 10 residential clients)Aide clients in establishing daily schedules and routines; help client develop other independent life skillsFacilitate the development of financial literacy and budgeting skills with the residents.Maintain bi-weekly case notes and weekly progress notes as mandated by Guest House and the Virginia Department of Corrections (DOC) policiesMaintain case files and other required documentation according to agency and DOC policiesRegularly identify and maintain relationships with new employment, housing, medical, and other community resourcesCommunicate regularly with relevant stakeholders as part of a resident's treatment team (eg, Probation Officers, Resident Specialists, etc.)Facilitate psycho-social groups as part of the Workforce and Life Development program scheduleOther duties as assignedPotential supervisory functions 1. Serve as administrator for the client database system2. Regularly run reports to ensure the timely completion of required documentation (case notes, service tracking, etc.)3. Assist with grant writing and reporting as required by Director of Marketing and Development4. Maintain the safety regulations of facilities as mandated by DOC and Guest House5. Collaborate with the Intake Coordinator to approve or deny former applicants requesting to return to the residential program; advise on other complex application issues6. Facilitate the internship program, training and onboarding for all Residential and Aftercare interns7. Determine caseload assignments based on client needs and Case Managers' expertise9. Provide feedback and input on disciplinary actions for residents, as needed10. Assist in organizing and scheduling trainings for case management staff11. Assist in Residential program development and restructuringGeneral 1. Enforce compliance with Guest House rules: drug and alcohol policies, emergency procedures, etc.2. Assist with fundraising events.3. Maintain confidentiality at all times.4. Attend and participate in staff meetings and in-service training.5. Inform supervisor of client and facility needs and concerns.6. Work closely with management to avert or resolve problems related to both the clients and the facility.Licenses & Certifications Valid driver's licenseCPR certificationFirst aid certificationQualifications This position requires a Master's degree in a relevant human services field such as social work, psychology, criminal justice, etc. Five years of professional experience in a human services setting assisting at-risk populations with histories of addiction, mental illness, homelessness, domestic violence, severe trauma, etc.) is preferred.Job Type: Full-timePay: $45,000.00 - $50,000.00 per yearSchedule:8 hour shiftMonday to FridayOn callEducation:Bachelor's (Required)Work Location: One location
