Case Manager - HBCAC Program Rockville, Maryland General Description: Dynamic position working with homeless men that requires an individual with housing first and trauma informed care knowledge. Skilled worker in motivational interviewing and able to meet clients where they are and move them quickly into permanent housing. Essential Duties and Responsibilities: Assists clients with moving onto permanent housing by providing targeted case management services. Maintain an ongoing case load. Ensures all residents have access to case management. Works closely with the entire case management team including the Program Director, Assistant Director, Housing Locator, and Employment Specialist to ensure effective operation of the program and facility and the integrated delivery of case management services. Assist in compiling all data for reporting purposes, including entries into the Homeless Management Information System Service Point. Maintain and review residents charts and files on a weekly basis to monitor progress and assist with moving onto permanent housing. Provide services to residents including assistance with benefits, identification, and provides referrals to community health services, psychiatric services, vocational, educational, and chemical dependence treatment programs, and referral to various housing options. Minimally meet with residents every week. Responsible for building and maintaining relationship with other service providers, including coordinating client service delivery with on-site providers of primary and behavioral health care. Attend and participate in the case manager meetings, monthly HBCAC staff meeting and other meetings throughout Montgomery County. Other related duties as assigned. Required Knowledge, Skills, and Abilities: Masters degree preferred in Social Work or Human Services field. Bachelors degree may be accepted with additional experience. Spanish language fluency preferred. Hands-on experience in social and human services field dealing with diverse populations. Knowledge of principles, methods and procedures of case management, housing first and trauma informed care Knowledge of principles, methods and procedures in handling addiction, mental illness, and Co- Occurring diagnosed populations. Good driving record. Ability to negotiate and maintain positive relationships with co-workers and residents. Advocate for homeless population. To Apply: Please apply on our website:
