Hiring ImmediatelyProgram Supervisors are responsible for the leadership, coordination, and implementation of all services offered to each individual while exhibiting a professional demeanor within the community. The supervisor will ensure that her team provides the most compassionate, least restrictive, community-based care for individuals to ensure their highest possible quality of life.Duties and Responsibilities include: SupervisionOrient new direct care staff to the home and their job duties in the home.Provide ongoing training and support to Direct Care Support Specialists and Residential Team Leader.Be familiar with GNCS policies and procedures and be able to address staff-related concerns appropriately.Secure staff to cover vacancies in schedule due to staff absences and no-shows. In the event this is not possible, responsibility for coverage lies with Residential Team Leader and Supervisor.Conduct and document monthly supervision meetings with Direct Care Support Specialists and Residential Team Leaders.Complete annual employee performance evaluations for all direct reports.Conduct two unscheduled pop-ups in the home (outside of scheduled shifts) on a monthly basis.Responsible for ensuring staff are knowledgeable on the Plan for Supports and support instructions.DocumentationCorrectly Purge each Individuals Chart, MAR, and House Book at the beginning of the month, and store it in an organized fashion in the home.At the beginning of each month, ensure that all paperwork is in the correct section of each individual's book after the previous month has been purged out.MedicalPhysician's Orders are in the medical visit books, transportation books, and MAR's.Ensure there are extra copies of all necessary paperwork (Med visit forms, backup documentation, Workers Comp paperwork, paper MAR, care-coordination notes) available in the homes.Conduct daily audit of Individuals charts, documentation, and MAR. Call staff to make corrections as needed.Review and audit employee time cards on a daily basis.Complete all necessary Annual paper work which includes Functional Assessment, Residential Directive, and Essential InformationReview and approve Activity Calendars and submit to Residential Regional Manager by the 20th of each month.Activity Calendars and Menus are posted in the home and emailed to Case Manager and client family members by the first of each month.Complete and submit Person Centered Reviews to the designated Program Support Specialist ten days before they are due.Complete billing and submit to finance department by designated due date.Fax Person Centered Reviews and updated physicals to the Case Manager the day they are dueFax Physician orders to Case Managers and other service providers (ie Day Support, CC/CE, etc).Payroll review and approvals are completed by 11am on Monday of payroll week.Emergency drills completed monthly.Physical Environment inspection sheet is completed quarterly and kept in the house book.Fire tags are initialed on the first of each monthUpdate Individuals paperwork (facesheet, emergency sheet,etc) when needed.Conduct weekly phone calls to Case Managers, Families, Legal Guardians, etc.Complete care coordination notes for important phone calls.Send out emails to administration when there are incidents or behaviorsProvide supervision of individuals and document supports.Assure new employees have been through the required training and been trained on each section of the individuals chart.Ensure that doctor's appointments and other appointments are scheduled and adhered to.Ensure that individuals are transported safely to their medical appointments.Ensure that the needs of individuals are advocated for during medical appointments, and the medical visit forms are signed and completed.Ensure that information are gathered from appointments, and are accessible to medical providers at the time of each visit.Update Person Center Reviews after the doctor's appointments using the medical visit form.Ensure that monthly vital signs are completed appropriately.Review and update electronic MAR as needed.Check bubble packs and cards on a weekly basis for expired medication and medication that was not administered.Ensure that medication closet/cabinet is clean.Ensure that individuals are educated about medications.Ensure that controlled medications are documented and counted for each shift.Responsible for the overall physical appearance of the charts and MAR (inside and out).Go through the monthly bulk medication from the pharmacy to ensure everything the medication is accounted for and the MAR's and PO's are printed correctly.Physical EnvironmentEnsure that individual's bedrooms, bathrooms, kitchen, and common areas for cleanliness that meet Medicaid requirements.Ensure that the physical environment is cleaned and maintained.Maintenance issues are communicated to the appropriate leaders in a timely manner.Ensure that the van is routinely maintained, and that the exterior and interior of the vans are clean.Ensure that the house is fully stocked with groceries, cleaning supplies, and office supplies.Ensure that quality food is available at all times.Ensure that all foods are appropriately labeled and dated.Individual SupportSupport individuals according to Plan of Support goals and objectives.Provide on shift coverage in lieu of a call out, behavior, or medical emergency.Responsible for responding to incidents after hours.Provide safety and support to individuals during daily activities.Ensure that staff know exactly what activities they have scheduled for each day and that they have whatever funds needed to do activity.Ensure that individuals receive their allowance on the designated days.Ensure that menus are properly being implemented, and that individuals are served meals/drinks according to their diet orders.Ensure that all individuals have neat, clean and weather appropriate attire at all times.Ensure all individuals are properly shaved, bathed, and groomed everyday(including, but not limited to toe/finger nails clipped, teeth brushed, face washed, etc).Customer ServiceProvide 1st Class Customer Service to all Families, Guardians, Authorized Representatives, and Case Managers.Will notify Residential Regional Managers if an individual does not attend day support and notify Logisticare immediately if needed.Will notify Residential Regional Managers if a Case Manager or parent calls with an issue or concern.Perform other work as needed by the Residential Development Manager.Ensure that overall physical appearance is neat and presentable (including; business casual attire, hair, neatly shaved, etc.).Absolutely No Smoking on company grounds.Responsible for explaining / conveying any pertinent information to the Residential Leadership Team.MINIMUM EDUCATION AND EXPERIENCEThe employee must have a Bachelor's Degree in the Human Services field or five years of related experience in the Human Services field.MINIMUM SKILLS, KNOWLEDGE AND ABILITIESThe employee must have:Knowledge of administrative and clerical procedures and systems such as word processing, outlook, excel, managing files and records, designing forms, and other office procedures and terminology.Knowledge of the medical field which includes working directly with doctors, nurses, therapists, and pharmacists.Demonstrates organizational, time management, managerial, and leadership skills.Ability to be flexible.Skilled in Crisis Intervention.Possess a valid Driver's License and must have a driving record that meets GNH general liability motor vehicle insurance carrier requirements. Should at any time a current employee's driving status changes and fails to meet the insurance carrier requirements, the employee may be ineligible for continuous employment.
