Fauquier HealthThe Physical Therapist organizes and conducts medically prescribed physical therapy programs in the health care setting to restore function, prevent disability following disease, injury or loss of body part.Essential Functions Consults with other members of the rehabilitation team, physicians, and others as needed to select the most appropriate treatment. Ascertains appropriateness of treatment ordered and contacts referring physician with any concerns. Designs and implements treatment plans, adapting interventions to the patient's personal goals. Delegates treatment to the Therapy Assistant and Rehabilitation Technician with verbal and written patient information, as appropriate. Monitors treatments and tasks performed by the Licensed Physical Therapy Assistant and the technician. Ensures that treatment goals are objective, functional, and measurable. Attains and maintains knowledge of appropriate growth and development principles relative to the pediatric, adolescent, adult, and geriatric patient. Incorporates knowledge and principles into the care of various patient populations. Attends and participates in patient care coordination meetings, team, and family conferences, as needed. Prepares complete written evaluation for patient's chart using objective information and concise language.
