Job SummaryWorks in partnership with local communities to design, implement, and evaluate health promotion, prevention and education programs. Requires Registered Nurse licensure and typically requires BSN or other relevant degree and certification / licensure.Minimum QualificationsMinimum QualificationsBachelor's degree in Nursing or other relevant degree (ie Nutrition, Dietetics, etc.)Possesses and maintains Maryland RN licensure or other relevant license (ie RD, LD)1 year experience in adult learning.Primary Duties and ResponsibilitiesContributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.Facilitate/teach (CDE) Diabetes Education and Self-Management Courses.Provide individual or group counseling within licensed scope of practice.Coordinate/facilitates bi-annual Diabetes Fairs.With hospital Case Management services, establishes transition plans for patients who are being discharged from the hospital.Participates and demonstrates competency in Electronic Medical Record (EMR) initiatives.Demonstrates knowledge of and participate in the development and promotion of clinical practices following MedStar St. Mary's Hospital observed Standards of Care.Actively participates in Quality Improvement Processes as a basis for assisting with the implementation of changes in practice.Performs various health screenings and supports general health education programs.Performs other duties as assigned
