OverviewCome work with a great team! Signet Health manages the behavioral health services for MedStar Washington Hospital Center. Our focus is to link physical and mental health for patients with a serious mental illness, older adults, and those suffering from multiple comorbid medical problems. We have openings for Full-time and Part-time, Days, Evenings and Weekends for: District of Columbia Psychologists, LICSWs or LPCs. Services include: Social work consultation & peer review participation Individual, group, and family therapy Interdisciplinary treatment planning Crisis intervention Adhere to: Joint Commission and other regulatory standards for clinical documentation APA and/or NASW Code of Ethics and exemplifies the qualities and values of the social work profession These functions are performed in accordance with all applicable laws and regulations, as well as Signet Health and MedStar Hospital's philosophy, policies, procedures, and standardsDuties Provides therapeutic clinical services to identified patients and families in the Outpatient Clinic, including individual and group counseling regarding the impact of illness and hospitalization. Collaborates with other health care professionals within the interdisciplinary team and other levels of care through the healthcare system. Facilitates communication regarding relevant psycho-social issues of patients and families to health care teams through timely recording in medical charts, consultation and participating in meetings. Functions as a coordinating link between patient, family, hospital, and community resources to assure continuity of care. Performs/documents psychosocial assessment. Provides liaison function to improve continuity of care and transition quality improvement, discharge planning, and social service departments. Serves as a resource for patients with psycho-social issues and provides a link to psychiatric resources. Participates in discharge planning process. Attends regular interdisciplinary team meetings for the operational and administrative functions of the outpatient care service. Performs other duties and responsibilities that are appropriate to the position and area.
