The Human Service Worker II (Family Assistance Worker) interviews applicants in order to collect financial, social and employment information to determine eligibility for Health Department essential services, Women, Infant and Children (WIC) program services, and connect them to other medical home to include local Federally Qualified Health Centers (FQHCs). After interview, registers and authorizes these services. Assesses need for other non-agency services; counsels clients and coordinates agency and non-agency services. Collaborates with medical/nursing staff.

Employment Standards

MINIMUM QUALIFICATIONS:

Any combination of education, experience, and training equivalent to the following:

Graduation from an accredited four-year college or university with a bachelor's degree, plus one year of professional work experience in human services or a related field.

CERTIFICATES AND LICENSES REQUIRED:

Must have a valid driver's license.

Cardiopulmonary Resuscitation - CPR (Required within 60 days)

AED (Required within 60 days

NECESSARY SPECIAL REQUIREMENTS:

The appointee to this position will be required to complete a criminal background investigation, a Child Protective Services check, a driving record check and a sanction screening to the satisfaction of the employer.

All newly hired employees are required to be fully vaccinated against COVID-19 (two weeks after the last required dose) as a condition of employment or obtain approval of a medical or religious exemption prior to their start date. Proof of an exemption or vaccination status will be required during the pre-employment onboarding process. New employees who obtain an exemption from the vaccine mandate for medical or sincerely held religious beliefs will still be subject to the weekly testing requirement. Vaccinated employees and employees with a medical or religious exemption will complete the attestation online on their first day of employment or shortly thereafter.

PREFERRED QUALIFICATIONS:
Preferred candidates will have a Bachelor's degree in Human Services, Social Work, or a related field. Experience conducting eligibility determination for public assistance for low-income ethnically diverse populations. Proficient in using an Electronic Health Care record System (EHR). Experience working with a diverse multicultural population. Must have a high level of integrity and discretion in handling confidential information and professionalism in dealing with clients and colleagues. Position requires candidates to work independently and in a team environment, exercise good judgment, use critical thinking skills, and have excellent communication skills. Bilingual in Spanish and English a plus.

PHYSICAL REQUIREMENTS:
Job requires walking, standing, sitting (for long periods of time), kneeling, reaching, bending, climbing stairs; uses hands to grasp, handle, or feel. Visual acuity is required to read data on a computer monitor. Ability to operate keyboard driven equipment and computer. Ability to drive a motor vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. Employee may be required to lift or carry up to 25 lbs. occasionally. Generally works in an office environment yet may occasionally be required to perform job duties outside of the typical office setting. All duties performed with or without reasonable accommodations.
