Position Information
The Aging and Disability Services Division (ADSD) within the Department of Human Services (DHS) is seeking an experienced Human Services Clinician for the Adult Social Services Unit to provide care coordination services for vulnerable adults aged 60 years and older and 18 years and older with a permanent physical or mental disability.

The Adult Services (AS) and Adult Protective Services (APS) programs are part of the Aging and Disability Services Division (ADSD). The Adult Services and Adult Protect Services units facilitate independent living in the least restrictive setting possible for older adults and adults with disabilities and prevent and reduce abuse, neglect, and exploitation of vulnerable adults. Both programs are mandated by Virginia State Law.

This team provides intensive community-based case management to: (1) ensure the safety of vulnerable and older adults; (2) prevent inappropriate institutionalization; (3) assess and assist with appropriate institutional placements; and (4) assist individuals maximize self-sufficiency. If the individual needs and accepts services, workers may arrange for a wide variety of health, housing, social, or legal services to stop the mistreatment or prevent further abuse. Assistance may include home-based care, transportation, adult day services, nutrition services and legal intervention to protect the adult. Services may also be arranged for individuals in emergency situations who lack the capacity to consent to services.

The Adult Social Services is part of the Aging and Disability Services Division (ADSD) that strives to prevent abuse, neglect, and exploitation; ensures persons remain safely in their homes when possible; and facilitates linkages to internal and community resources. ADSD is part of an integrated social and wellness model, connecting customers to wrap around supportive services including adult protective services, adult social services, home-based nursing case management, in-home services, Medicare counseling, senior adult mental health, and volunteer guardianship. The population served is culturally and ethnically diverse.

Specific duties include:

  • Conducts face to face visits with clients within specific timeframes, coordinates home and community-based services and collaborates with other health and safety and service professionals in the community as well as families and friends of clients;
  • Completes comprehensive assessments using the Virginia Uniform Assessment Instrument (UAI) to evaluate need for services, risk, and to assess physical health, mental health, cognitive functioning, behavior, functional health, environment, financial situation, and formal/informal support systems;
  • In collaboration with the client, develops a client centered, solution focused care plan to prevent/ameliorate abuse, neglect, or exploitation. Care plan development may include obtaining input from multidisciplinary teams. The care plan includes development of formal and informal support systems.
Responsibilities include:

  • Performs comprehensive assessments in both the office and/or place of residence to determine need for resources and services;
  • Prepares a complete client service plan to remedy unmet needs and reduce behavior patterns of risk, provides crisis stabilization to reduce and/or eliminate the risk of abuse, neglect / self-neglect and/or financial exploitation, and solicits family/caregiver cooperation in care planning;
  • Evaluates periodically the progress of the adult or living environment towards meeting objectives of the service plan, the need to modify the plan, and the eventual closing of the case;
  • Completes and participates in court processes, including but not limited to guardianship/conservatorship hearings;
  • Offers alternatives to living environment if the client has been deemed incapacitated which may include placement of the adult client in an alternative living environment;
  • Maintains electronic case records in compliance with applicable clinical and regulatory requirements; and
  • May conduct investigations concerning allegations of abuse, including sexual abuse, by talking with and visually observing older adults and persons with disabilities; talks with immediate family, relatives, neighbors, healthcare providers, and other relevant parties to determine validity and need for services.

The ideal candidate will also be unprejudiced and adapt at functioning within a multi-disciplinary environment that believes in applying strengths-based and person-centered approaches to the delivery of human services.

Selection Criteria
Minimums:

  • Master's Degree in Human Services, Counseling, Psychology, or other directly related clinical discipline, all of which required a practicum prior to graduation involving case management services and/or treatment services; and
  • Two (2) years of post-Master's degree experience providing professional clinical human services to older adults, which includes the application of case management services and/or treatment services.
Substitution: A directly related higher level clinical degree may substitute for the Master's degree education requirement and one year of professional clinical social work experience to older adults. In the case of such substitution, the remainder of the qualifying work experience must have been gained after the higher-level degree was awarded.

Desirables: Preference may be given to candidates with experience in one or more of the following:

  • Providing services to persons with disabilities, older adults (60+), and their families;
  • Working with Adult Services, APS, individuals with SMI and/or experiencing homelessness, or other case management practice;
  • Collaborating with Courts and the legal system when providing services;
  • Working with individuals from diverse cultural/ethnic backgrounds.

Special Requirements
Applicant must possess, or obtain by the time of appointment, a valid driver license, or the applicant must have the ability and willingness to use alternative methods of transportation to perform assigned duties at locations other than the primary worksite. If the applicant possesses or acquires a license, the applicant must provide or authorize Arlington County to obtain a copy of the applicant's official state/district driving record. Any offer of employment may be contingent on a favorable review of the applicant's driving record.

A background check and State of Virginia Child Welfare and Neglect Central Registry may be conducted on all candidates who are finalists for the position. It may include checks of the following: criminal record, driving record, education, professional licensure, and credit check and may require signing a release authorizing the County to obtain this information.

The position will require travel primarily in Arlington County.
