General Summary Examines and treats patients independently and in autonomous collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The Nurse Practitioner works within a collaborative model to provide quality care and to improve patient outcomes by strengthening continuity of care in all settings for patients, families, and staff in both the hospital and physician practice settings. Essential Functions & Accountabilities 1) Accurately performs history and physicals for patients. Utilizes sound clinical judgment based on best practice when implementing the therapeutic regimen. 2) Documents all care provided, including pharmacological dispensing, in an accurate manner. 3) Completes patient discharge planning and education; writes appropriate orders and prescriptions. Performs patient education and counseling to promote wellness. 4) Evaluates patient's progress toward attaining expected outcomes. Evaluates effectiveness of interventions in relation to outcomes. Documents patient's response to interventions. 5) Maintains accurate, legible, and confidential medical records in accordance with federal and state laws. 6) All other duties as assigned, within the scope and range of job responsibilities. Minimum Education and Experience Minimum Preferred Education: Master's degree in area of specialty. Certification as a Certified Registered Nurse Practitioner (CRNP) and state authority for advanced practice in Adult Health. Experience: Two (2) years' experience as an Nurse Practitioner in a physician practice setting and/or related nursing experience Three (3) years Nurse Practitioner experience in specific area of practice (ie Oncology office). Computer/Software Knowledge: Basic skills with Microsoft Office programs, or similar programs, and patient database. Able to navigate medical websites Advanced computer skills; skilled in use of EMR (electronic medical record) in a medical office environment Licensure/Certification Current Delaware Advanced Practice Nurse license with prescriptive authority. BLS and ACLS required. AOCNP eligible with completion of certification within 2 years of hire. AOCNP certification.
