Our licensed Case Managers are called "Clinical Care Coordinators" and provide these services for our clients in different programs: Develop individualized assessments and plans of care Provide direct community-based resources for clients based on their individual needs On-site visitation to clients in the community Telephonic support and monitoring of services As a teleworking organization, we provide the flexibility to work from home while giving you the support you need to manage the needs of our clients with complex medical needs. And best of all: no evenings or weekends are required in this position! Our Clinical Care Coordinators visit clients throughout their surrounding county/region to support their needs in the community, with mileage reimbursement available. We work with clients of all ages and have a collaborative approach to training, mentoring, and professional development.
