Rock Creek Foundation is seeking a Full-Time Delegating Nurse in Silver Spring, MD to support the individuals in our DDA program with severe and persistent mental illness and intellectual disabilities in accordance with COMAR, State of Maryland Developmental Disabilities, and Maryland Board of Nursing. Essential Functions: Administers hypodermic injections, following Physician's prescriptions and agency procedures Responds to medical emergencies and triages accordingly to determine appropriate level of care Completes all initial health/nursing assessments for current individuals and for new clients prior to admission in the residential program Performs all 45-day reviews for individuals including taking vitals, reviewing past and current records, reviewing medication administration records, PMOF, and ensuring medical appointments have been completed Provides additional Nursing Support hours per identified individual(s) Service Funding Plan for more intense medical diagnoses/conditions global and/or individual specific needs Reviews all doctor orders on a monthly basis, as well as monthly cycle medication prior to the 1st of every month Takes lead on hospital discharge planning and coordination of care prior to the individual returning to community residential setting Completes and updates the HRST State-wide system and initiatives Provides case management activities for coordination of care with primary physicians, internal/external psychiatrists, hospitals, and agency appointed pharmacies Acts as a consultant and educator to staff, individuals and families regarding health issues, medication and medical conditions Provides CMT supervision and on-site medication observation, as well as quarterly clinical CMT renewals Attends annual IP meetings Purging of medical records on a quarterly basis or more often if needed Maintains client caseload and provides treatment coordination according to all applicable regulations and policies Ensures safe storage of all medications and medical supplies according to regulations and agency policy at each licensed site Partners with Administrative and Management teams to ensure agency's compliance with all State and agency regulations, policies and procedures Required Qualifications: Must possess an active Registered Nurse (RN) License in the state of Maryland that is in good standing Bachelor's Degree in Human Services or related field of study Valid Driver's License with no more than two points Proof of current automobile Insurance Preferred Qualifications: CPR Certification strongly desired Prior experience as a Delegating Nurse (RN) in a behavioral or mental health setting A strong desire to care for our individuals and act with integrity Must be able to work with "high-risk" consumers who have mental illnesses as well as developmental and physical disabilities. New hires who show proof of their completed COVID-19 vaccination will earn a sign on bonus on their first paycheck.
