Currently seeking case manager who will work on the patient's behalf to oversee each patient's individual case. The case manager will communicate with patients, providers and insurance companies, providing a single point of contact for the patient, serving as liaison for the patient with hospitals, providers and insurance companies. As the patient liaison, the case manager will keep a file on each patient with specific information, current status and upcoming requirements. The case manager will provide the patient with current information on medical status and any pertinent clinical instructions. The case manager will also provide the patient information on upcoming treatment recommendations, medications, activity limits and decisions that must be made.The Case Manager will communicate with the patient and provide information regarding current medical status and clinical instructions regarding recommended treatments, medications and activity limits for the patient including the discharge instructions regarding departure and pick up from the hospital. The case manager will assist the patient on discharge options including pickup times and potential transportation challenges that must be addressed prior to discharge.For patients moving to another facility, the Case Manager verify insurance eligibility and coverage for the specified care at the receiving facility and inform the patient available benefits, services and other resources. This includes verification of insurance eligibility and coverage for the specified care at the receiving facility. To ensure the patient receives complete care, the case manager will review all available benefits and portions the patient will have in the facility they are transitioning to.
