Someone who is innovative and a self-starter is a must! This position will oversee Care Coordinators working with members who receive Home Care based services. The Manager will ensure compliance with the contract requirements for the assessment and care of the population, as well as provide strong, encouraging leadership to the team.$5000.00 SIGN ON BONUS FOR EXTERNAL CANDIDATESPrimary Responsibilities:Demonstrates UHG cultural values, including modeling ethical behavior, delivering value to customers, leading and developing people, leading change and innovation, driving sound disciplined decisions and driving high quality executionEnsures compliance of Case management and coordination of care per policies, procedures and contractual requirementsEnsures consistent leadership and direction for teamSets team direction, resolves problems and provides guidance to members of the teamTakes lead role in setting direction and participating in or developing new programsClinical operations across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating)Interprets complex information, simplify, clarify and communicate effectively to othersEnsures team meets established performance metrics and performance guaranteesAdapts departmental plans and priorities to address business and operational challengesYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:BSNCurrent, unrestricted RN license in DC3+ years of experience in a hospital setting, acute care, direct care experience or preferably in field-based care coordination2+ years of Supervisory/Management experienceProficient with Microsoft Office, with the ability to navigate a Windows environmentResident of DCPreferred Qualifications:Case Management experienceExperience in utilization review, concurrent review and/or risk managementHome Health experienceDC Medicaid knowledgeBackground in managed careExcellent verbal and written communication skillsSolid organizational skills and multitasking abilitiesFull COVID-19 vaccination is an essential requirement of this role.
