The TCM Care Coordinator Youth and Children reports to the TCM Supervisor and provides service to youth ages 0-21 who have a behavioral, developmental, medical, or mental health need who are in need of services in order to remain in their home and in the community. The TCM Care Coordinator is responsible for developing and organizing Child and Family Teams (CFT) to assess and meet the multiple needs of children who have or at risk of developing behavioral and emotional problems. RESPONSIBILITIES Facilitates Child and Family Team meetings at which the team identifies strengths and needs across all relevant life domains and organizes, initiates, reviews, and revises documented planning within 30 days of a case assignment and then 90 days of case. Coordinate services such as access to assessments and treatment services and across the multitude of systems with which the individual is involved and finally coordinates care with child welfare and other community organizations concerning placement determinations and other therapeutic services. Perform 75% of the time towards engagement in activities out in the community and/or the consumers' natural setting. Develops individualized service plan for each consumer within 10 days of case assignment. Ongoing assessment and reviews of treatment progress are performed on a regular basis. Documents all encounters with clients and families within 24-48 hours. Works closely with Community Support Specialist/TCM Supervisor in individual and group setting. Develops strong community relationships and resources in support of consumers as part of treatment. Will attend and complete all mandatory trainings assigned by TCM Supervisor. Will complete any other duties assigned by the TCM Supervisor EFFECTS ON END RESULTS Improvement or increased stability in the consumers level of functioning. Ensures that each consumers individualized needs are met. Ensures that all documentation pertaining to consumer services is completed in compliance with agency policies and funding source requirements. Presents a positive image to the consumers and public. Ensures compliance with contractual obligations. PHYSICAL REQUIREMENTS The physical requirements described below are representative of those that must be met by an employee to successfully perform the essential duties of this job. The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary. The ability to bend, twist, and lift up to 35 pounds. Operating a vehicle requiring continuous or repetitive hand/arm movements. Minimum two (2) years successful experience working with children and youth with serious behavior or emotional mental health issues. Experience with in person meetings/services are required in the home or community for relationship building Develop effective working relationships with the consumer population and community partners. Self-motivated and ability to motivate others. Excellent organizational and follow up abilities. Excellent verbal and written communication skills. Computer proficient in Microsoft Office suites. Bilingual in any language a plus
