Adventist HealthCare seeks to hire a Manager, Post-Acute Services Center who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Manager, Post-Acute Services you will:
•Managers the overall operations of the Post-Acute Services Center
•Educate, train and provide overall support and mentoring to the Post-Acute
Services Center staff to include pre-admission intake documentation into EMR
systems, insurance benefit verification, patient scheduling, organization of
transportation to acute rehabilitation from acute care hospital, referral
submission to post-acute services outside of the AHC network when necessary
•Coordinates and facilitates overall functions with Business Development
related to current and future referral sources.
•Ensures support and back up for all Post-Acute Service Center staff as
necessary to ensure department goals are achieved.
•Track weekly performance, productivity, and analyze referral trends while
upholding the values and customer services standards of the organization.
•Collaborates closely with Care Navigation Teams to ensure processes are
integrated and patients are transitioning smoothly to the appropriate Post-
Acute Services setting
• Provides managerial direction to the Supervisor, PAC Center

•Bachelor's Degree required
•Minimum 8-10 years experience in case management, acute rehab, home
health or other health care setting
•3-5 years supervisory experience preferred
•Valid Maryland RN preferred
