Seeking a highly skilled manager to oversee the Health Department Long Term Care (LTC) services. This includes oversight of four (4) Adult Day Health Care Centers (ADHC), and the LTC Medicaid screening teams. Management duties include budget development, implementation and monitoring, program and policy development, quality improvement initiatives, program performance management, resource allocation, staff development and performance management, strategic planning and project management. This role requires the ability to pursue and maintain multisector professional relationships. The LTC Coordinator is expected to analyze data and conduct community assessments to identify emerging needs, and then to engage in collaborative strategic planning to meet those needs. Provides direct supervision and development of professional staff. The Health Department is looking for a professional who can work on a multidisciplinary team to promote an integrated system of services for older adults and adults with disabilities in the county.

Employment Standards

Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in nursing; plus five years of post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education, including three years in a public health/community health setting. Qualifying experience may include experience in ambulatory care settings such as outpatient clinics that incorporate community outreach/education as part of services or specialty settings that incorporate teaching, community outreach, resource referrals, etc. as part of a health care continuum; Plus at least one year of supervisory experience.

  • Possession of current license or a multi state licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia.
  • Valid motor vehicle driver's license.
  • Cardiopulmonary Resuscitation (CPR) Certification required within 60 days
  • AED certification required within 60 days

The appointee to this position will be required to complete a criminal background investigation, a Child Protective Services (CPS), a driving record check, and sanction screening to the satisfaction of the employer.

All newly hired employees are required to be fully vaccinated against COVID-19 (two weeks after the last required dose) as a condition of employment or obtain approval of a medical or religious exemption prior to their start date. Proof of an exemption or vaccination status will be required during the pre-employment onboarding process.   • Preferred candidates will hold a master's degree in public health, public administration, nursing, or a related degree.
  • Possess strong program management skills.
  • Experience working with older adults with cognitive and physical impairments (i.e. dementia).
  • Data analysis and budgeting experience.
  • Strategic planning skills, public speaking, team building and conflict management skills.
  • Exceptional communications skills both orally and in writing.
  • Extensive experience in the use of Microsoft Office computer software, specifically Word and Excel.
  • Project management experience.
  • At least five (5) years of direct supervisory experience overseeing, monitoring, and directing the work of others.
  • Proven team leadership skills with a commitment to development of staff.

Employee is required to walk, stand, sit, bend, kneel, reach, climb stairs and carry equipment or young children up to 25 pounds in weight. Uses hands to grasp, handle or feel. Visual acuity is required to read data on a computer monitor. Ability to operate keyboard driven equipment. Ability to drive a vehicle. Ability to assist with client transfers from sitting to standing. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing.
