What You Will Do: The goal of the Clinical Liaison is to identify potential clients who meet inpatient rehab criteria, assess and facilitate effective care transitions to the rehabilitation setting. Job duties include but are not limited to the following: * Demonstrates expertise in analysis, decision making, time management, oral, written and digital communication. * Understands regulatory requirements, resources available and parameters of third-party reimbursement. * Provides early identification of potential rehab candidates through collaboration with the clinical team at the acute care hospital by attending multidisciplinary rounds and communicating with physicians, therapists, case management, and nursing staff. Assists in determining readiness for acute rehab program participation. * Performs baseline and ongoing assessments of potential patients. This includes gathering information on medical history, tolerance to therapies, potential for improvement, and discharge plans. * Understands rehab diagnoses, prognosis and clinical indicators in order to make an appropriate decision regarding admission to acute rehab. * Interacts clearly and professionally with patients, families and other healthcare professionals while gathering additional information to promote the rehab unit * Interprets the medical record to determine medical stability for discharge to inpatient rehab and verifies completion of the planned medical work-up. Collaborates with case management to ensure any outstanding medical work up is scheduled to expedite rehab admission. * Completes a patient preadmission assessment prior to patient admission with physiatrist approval and obtains the appropriate signature page for the medical record. Clarifies additional clinical information as requested by the physiatrist. Reassess patient for changes in plan prior to admission and communicate necessary information to physiatrist and rehab clinical staff. * Identifies trends in medical parameters and anticipates the need for follow up labs and testing. Communicates trends to the physiatrist as indicated and facilitates follow up medical testing with physician/case manager at discharging facility as needed to expedite rehab admission. * Serves as a rehabilitation nurse consultant, with attention to the client's rehabilitation potential, therapy and functional progress, discharge plan and feasibility, special equipment needs, rehabilitation services across the continuum of care and care transitions. * Educates patients, family members or significant others, referral sources, interdisciplinary team members, external payers, and external case managers about acute rehabilitation benefits and services. * Develops and fosters positive relationships with ongoing and potential referral sources including care management, physicians, payers, acute care hospitals, skilled nursing facilities, LTACHs, and home health agencies. Anticipate and monitor changes in market trends and referral patterns. * Maintains working knowledge of insurance authorization and benefit requirements for rehabilitation services for program, facility and payer. Assists in determining benefit eligibility and precertification, including providing a plan of care. Acquires and disseminates accurate payer information to facilitate the admission process, clarify any uncertainties and finalize authorization for admission. * Facilitates tours for residents, families, case managers, discharge planners and physicians of Winchester Rehab as requested. What You Will Bring: Education Bachelors Nursing (BSN) required Experience 5 years in Healthcare/Medical - Nurse: LPN, LVN required A minimum of 3 years of clinical experience; experience in the rehabilitation of chronically or catastrophically ill or injured individuals.
