As a Long Term Care (LTC) Clinical Manager, you will provide 24 hour management support of specified patient populations for optimal patient outcomes. To help achieve our mission, you will facilitate the provision of safe and therapeutic care in a holistic and systematic way to a specified patient population.Integrating your knowledge, skills and experiences to meet the needs of patients and families throughout the continuum of care with an emphasis on patient/family education is of vital importance. Your ability to ensure improvement in practice settings by assuming responsibility for self-development in lifelong learning is expected. Providing direction and guidance to others regarding practice while serving as a resource and mentor is required. Duties and Responsibilities*Ensures quality patient care by management of clinical nursing practice and patient care delivery that supports optimal patient outcomes.*Facilitates the multidisciplinary team and incorporates their expertise to achieve positive patient outcomes.*Formulates an individualized plan of care for each patient with needs to achieve desired outcomes.*Recognizes behaviors that impact team function and implements strategies to optimize teamwork.*Participates in The Joint Commission and state survey visits.*Develops fiscally responsible capital budgets that meet department equipment needs. Analyzes and reports fiscal variances while developing an effective corrective action plan for all fiscal variances.*Manages clinical nursing practice and patient care delivery through the planning and implementation of programs/processes that support optimal care outcomes.*Ensures patient care compliance with professional regulatory/government standards of care. Evaluates staff on practice delivery and initiates education as needed.
