We work innovatively to bring new services, ideas and ways to care for our patients American Care Partners is currently seeking a dedicated LPN to join to our Exceptional Clinical Team who can provide quality care to our patients, under the supervision of the RN Case Manager. Duties and Responsibilities: Private duty nursing for Pediatric and Adult clients in home and community setting Provide appropriate care in accordance to physician orders Ensure the well being of the patient Required Skills: Good documentation skills G-Tube - NG-Tube - J-Tube Ventilator experience Tracheotomy experience Punctual, very caring, and compassionate Qualifications: Updated CPR Valid Identification Current, unencumbered state license to practice as an LPN Social Security card (or work order/visa) Copy of PPD or Chest X-ray Copy of Immunizations of Hepatitis B and Influenza Copy of updated TB results Requirements: Minimum of 1 year experience as an RN/LPN; Home healthcare preferred Minimum of 1 year experience with GTube, Trach, and Vent Graduate of an accredited Licensed Nursing Program.
