Summary

The Salem VA Medical Center is looking for a Home Telehealth Coordinator (RN) to join their team. Position will be located in Salem, located on station and will be required to be available to meet Veterans for screening in Primary Care, specialty clinics, or on the inpatient wards to discuss home telehealth. If the Veteran meets criteria for the Care Coordination and Integrated Case Management program, the Care coordinator may be asked to take a lead role in managing the needs of the Veteran.

The Home Telehealth Care Coordinator is responsible for the assessment, intervention, periodic evaluation, and discharge of Veterans that are managed through virtual applications including home-based equipment and telephone. The Home Telehealth Care Coordinator provides care management through technology that increases the likelihood that a Veteran will receive accessible, coordinated care. He/she provides professional assessment, coordination, and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received, and that progress is being made. The Home Telehealth Care Coordinator is a member of a team who uses innovative telecommunication technologies to enhance care coordination and to proactively manage chronic diseases to prevent healthcare crises. The goals of the Home Telehealth (HT) program is to improve chronic disease management, improve patient satisfaction, increase access to services and reduce resource usage thus decreasing health care costs.

The Home Telehealth Care Coordinator will utilize a chronic disease management model, develop appropriate coordinated plans of care and effective treatment plans, educate Veterans and families on the tele-health model, monitor and evaluate Veterans using telecommunication technology, and arrange for changes in
level/type of care as necessary.

Duties and responsibilities for the position include but are not limited to the following:
  • The nurse is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care Uses sound clinical judgment in ensuring the delivery of care to the patient population.
  • Demonstrate grade appropriate and consistent critical thinking skills.
  • Anticipates and recognizes medical/psychiatric emergencies and initiates or lead other to implement appropriate and timely actions.
  • Documentation is technically informative, complete, accurate, consistent, concise, and timely.
  • Communicates effectively both orally and in writing with professional administrative and technical staff within the medical center and other organizations and/or providers as needed.
  • Demonstrates process improvement strategies to improve Veteran care.
  • Adheres to infection control practices while using equipment and providing patient care.
  • Handles multiple tasks in a fast-paced environment with frequent interruptions while maintaining clear concise communication with all involved parties.
  • Uses video-conferencing technologies to coordinate and connect staff, resources, Veteran, and providers in the manner effective for delivery of services, patient care, education, and training.
  • Instructs Veterans and family members on principles of health, self-care, and protocols for treatment and/or medications. Utilizes teaching materials to promote self-reliance and independence including methods to prevent complications and facilitate recovery. Reinforces instruction provided by other health care professionals.
  • Initiates instruction to patients regarding equipment, medication, treatment procedures and diagnosis. Reinforces instructions to patient/family that were previously presented by other team members.
  • Contributes to telehealth through process improvement activities to improve care.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/21/2021.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience:
  • 2-3 years of Case Management experience preferred but not required
  • 2-3 years Med/Surg experience preferred but not required


Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements:
The Home Telehealth Care Coordinator is required to meet the physical standards as outlined on the OF 178 Certificate of Medical Examination. The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The position requires long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required. Staff may be exposed to infected patients and contaminated materials and may be required to don personal protective equipment. The RN must be capable of working effectively in stressful situations and be able to shift priorities based on Veteran needs. BLS is required.

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
