Job DescriptionAssists in the delivery of direct patient care under the supervision of the Registered Nurse. Communicates and collaborates with care team members in implementing the plan of care to meet individualized and developmental patient care outcomes. Partners with Registered Nurse to create a professional development plan to provide employee with the necessary skills to deliver up-to date patient care. Utilizes information to continuously improve patient care and practice to enhance outcomes.Job ResponsibilitiesProvides basic activities of daily living nursing care (example: bathing, feeding, hygiene, toileting, ambulating) monitoring activity, sleep, and intake/output.Demonstrates competency in all required clinical skills and technologies. Examples include but are not limited to safety observations (Q15", 1:1), sharps maintenance, search and disposition of patient belongings, de-escalation and crisis prevention.Monitors patients and the environment, and intervenes appropriately to promote a safe and therapeutic milieu.Provides information to patients, families and visitors. (eg orients patient to the unit and program, explains rationale for rules or restrictions, provides printed information) .Completes assigned patient duties each shift. (individual patient care assignments, and assigned milieu tasks) Completes forms and flow sheets accurately according to policies and procedures. Reinforces transition/discharge teaching and home care skills. (Eg reinforces and models healthy behaviors, empowers patient to develop safety plan)Communicates patient's progress toward desired outcomes to the Registered Nurse. Communicates changes in patient status to the Registered Nurse in a timely manner.Identifies own learning needs/goals based on timely self-evaluation. Makes progress/meets goals set from the previous year. Attends educational and training opportunities.. Maintains own Employee Continuing Education Record and completes all annual competency reviews.Describes the unit's quality improvement activities, trends, and goals. Participates actively in the quality improvement initiatives.
