Responsibilities

The incumbent works for the Central Virginia VA Health Care System's Hospital and Clinics in Richmond, Virginia, as a direct report to Social Work Service, and is assigned to the Emergency Department. This position provides clinical social work services to patients in the Emergency Department, as well as support to social workers across the health care system to implement and increase telehealth visits when clinically appropriate and in alignment with VHA policies and goals. The incumbent also assists the facility's Suicide Prevention Coordinator with the management of the Safety Planning in Emergency Departments (SPED) program.

Duties may include, but are not limited to the following tasks:
  • Adhere to Medical Center Policies (MCP) and Standard Operating Procedures (SOP) for documentation. Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, CARF, and/or revenue sources providing funding to VHA.
  • Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in the discharge planning process both within the facility and for potential placement and/or aftercare programs/facilities.
  • Adhere to MCP, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post discharge follow up.
  • Provide individualized care coordination and discharge planning, highlighting patients strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts.
  • Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress.
  • Ensuring patients have access to care via completion of required documents, 1010s, Advanced Directive, and other forms that assist with access to care within the VA or the community.
  • Participate in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service and at the facility level as well (i.e. infection control and safety).
  • Tracking and reporting data for SPED and employee telehealth utilization.
  • Participate in continuous survey readiness through ensuring reports, PI projects, and other documentation required for accreditation are current and up to date.
  • Participate in all assigned coordinator meetings to include individual, team huddles, and will participate in, both social work and matrix, staff meetings.
  • Participate on all assigned committees, work groups within or external to the facility. Supervisory approval is required prior to working with a committee within or external to the facility. (See business compliance regarding non-VHA committee participation).
  • Participate in coverage for social work services on the assigned unit/service, and when needed for other social work teams, including inpatient and outpatient, as needed.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/18/2021.
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Policy.
  • Education. A master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. To verify that a social work degree meets the accreditation standard for a master of social work, access the CSWE website at http://www.cswe.org/Accreditation/organizations.aspx.   • Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Social work Coordinators must be licensed or certified by a state at the advanced practice level (LCSW or equivalent). (Copy of licensure)

Grade Determinations:
GS-12 Social Worker (Program Coordinator)
  • Experience and Education- One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification- Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities- In addition to the experience above, the candidate must demonstrate all of the following KSAs:
  • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to provide training, orientation, and guidance within clinical practice.
    • Assignments- For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole
      practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to
      enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and
      reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.]

    The full performance level of this vacancy is GS 12.

    Physical Requirements: The work is primarily non-strenuous though a significant amount of walking is required for face to face contact with patients, family members and medical staff. Ability to lift 5-10 pounds, mostly documents and/or binders is required. The ability to sit for 1-2 hours and do substantial computer work.

    Work Environment: Work is performed in an office/clinic setting with minimal risks that requires normal safety precautions; the area is adequately lighted, heated, and ventilated. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
