The Patient Navigator functions as a resource and advocate for African American pregnant and postpartum women with young infants up to 12 months in the Washington DC area, as part of a research project funded by the PCORI (Patient Centered Outcomes Research Institute). This position will: (a) conduct periodic mental health screenings, (b) systematically review screening data, (c) provide appropriate responses for identified mental health needs around social determinants of health, and (d) collaborate with all involved research teams and partners to ensure that patient and family needs are met. Ability to work with African American patient populations (preferred).

Qualifications

Minimum Education
Bachelor's Degree (Required)

Minimum Work Experience
2 years Social services or medical setting offering direct assistance to clients, patients, and their families. (Required)

Required Skills/Knowledge
Strong communication skills.
Ability to relate to diverse patient population.
Sensitivity to individuals from varied backgrounds and cultures.
Committed to the health care needs of children, adolescents, and their families.

Functional Accountabilities
Patient Support Activities
  • Staff inpatient Teen Room to provide a safe space for teen and young adult patients to relax, play, and socialize; coordinate, organize and monitor Teen Room activities and resources.
  • Facilitate participation in Teen Room activities; actively recruit for patient attendance.
  • Anticipate needs of teens and young adults on the floor and provide for those needs, as appropriate.

Program Support
  • Communicate observations to the health care team, psychosocial team, and direct supervisor.
  • Participate in psychosocial team meetings.

Ulman Fund
  • Execute Ulman Fund programs with excellence and adherence to mission to improve the lives of young adults impacted by cancer.
  • Compliment existing Ulman Fund programs with new programs and activities that enhance the patient and family experience.
  • Produce regular reports to Ulman Fund on overall program delivery, patient attendance; share stories, photographs and update as appropriate for social media outreach to demonstrate impact of program at Children's National.
  • Work closely with Director of Development to provide feedback to Ulman Fund.
