Fort Washington Medical Center seeks to hire an RN Care Navigator who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As a Case Manager/Care Navigator, you will:
•Complete comprehensive psychosocial assessment and review of clinical needs of patients with focus on patient's requirements as they transition to the next level of care
•Collaborate with physicians, social workers and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning and preventing delays in patient care; communicates barriers to leadership for resolution and trending
•Identify services and resources available in the community and assists with patient connection to these services
•Participate in Interdisciplinary Rounds and other patient care conferences
•Collaborate with Utilization Review staff
•Review medical records for clinical criteria, appropriate level of care and compliance with standards

Qualifications Include:
•Bachelor of Science in Nursing
•Minimum of 3 years experience in diverse clinical settings
•Licensed Registered Nurse in the State of Maryland
•Active American Heart Association Basic Life Support (BLS) certification required
•Prior case management or discharge planning experience preferred
•Prior experience with Cerner EMR and All Scripts is preferred
•Strong written and verbal communication, excellent customer service skills to support open dialogue with patients
•Exceptional interpersonal skills including collaboration and ability to relate to other disciplines
•Empathic toward individual's complex health needs and their families
•Effective use of time management skills to multitask, manage relationships with post-acute providers and gather resources for the department.
•Working knowledge of federal, state and local laws that govern health care and case management
•Knowledge of community resources
•Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
