Resp & Qualifications Position Summary: The position supports the Maryland Medicaid membership and reports to the Clinical Outreach Associate Supervisor. The Clinical Outreach Associate (COA) serves as a liaison between the health plan, healthcare providers and members. The target population COAs will address, are members in need of chronic condition education and care coordination. The goal of the COA is to connect members with the appropriate care related to their chronic condition needs and beyond. Essential Job Duties and Responsibilities: * Serves as a point of contact for health care services and referrals for members with medical and behavioral health needs. * Reviews encounter forms/claims to identify all potential case management service needs and collaborates with RN case managers, ensuring member-focused treatment planning, and addressing physical, behavioral and psychosocial needs. * Works closely with case managers, case manager assistants, and other care coordination departments, integrating case management to meet the needs in the bullet above. * Collaborates with members, families, and healthcare providers to enhance patient wellness and facilitate connecting members to care. * Maintains educational materials, assists with member mailings, remains knowledgeable of member benefits/resources, and supports the case management team in various other duties. * Monitors member compliance telephonically and is available to provide consultation as needed. * Develops, implements, and maintains case management support programs to facilitate the use of appropriate medical and community resources, and collaborate with providers. * Documents all activities in the appropriate system(s) in a timely way. * Assists, as needed, in quality initiatives for department. * Other duties as assigned in support of servinig the member popular and care coordination. Education, Experience and Qualifications: * High School diploma * 3 years of managed care or insurance industry experience preferred. * Experience in care coordination, case management, disease management, hospital/acute care setting, or customer service preferred. * Knowledge of State and Local regulations and community resources a plus. Knowledge, Skills and Abilities: * Exceptional communication skills, both written and oral, with the ability to positively influence others with respect to compassion (fluency in a foreign language is a plus). * Strong work ethic built on a foundation of reliability, promptness, proactivity, and teamwork. * Make decisions regarding work processes based on established guidelines. * Prioritize and organize work to meet changing priorities. * Compile, analyze, and organize data and information from multiple sources to carry out assignments. * Work independently with minimal supervision. * Possess proficient PC skills and demonstrate proficient usage of various software to include spreadsheets, databases, etc., In addition to the ability to use highly specialized/customized software for database management and preparation. * Able to set and meet deadlines, working independently, as well as on a team. * Personal and professional track record that demonstrates a commitment to quality in healthcare. * Willingness to "think outside the box", accept feedback, and embrace the challenges of implementing a new model. * A positive attitude: ability to work hard, have fun, and operate efficiently in a fast paced, start up, work environment and in a highly complex and dynamic movement for health delivery reform. * Use critical thinking and defined policies and procedures to coordinate and manage the flow of information within area of responsibility.
