Job Req ID: 59537 CO Nurse Practitioner The Department of Medicine, Division of General Internal Medicine is seeking a Nurse Practitioner (NP) for the outpatient medicine faculty practice located at the Johns Hopkins Outpatient Center (JHOC). The NP will work collaboratively to provide comprehensive and age appropriate care to both well and/or ill adult patients. In addition to caring for a small patient panel, the NP will provide urgent and ongoing care to patients empaneled by faculty primary care providers. Extended visit times and significantly lower visit volumes than seen in typical primary care practices are the standard in order for the NP to develop meaningful relationships with their patients and other primary care providers. The NP will collaborate and coordinate care with interdisciplinary care teams to ensure the plan of care is in line with and supports the patient's goals of care, physical and social needs. The NP will be responsible for documentation of visits and follow up re-assessment(s) in the electronic medical record, along with recommendations and/or prescriptions. Specific Duties & Responsibilities: * Work schedule includes 40 hours per week, being comprised of seven (7) in-person or telemedicine clinic sessions (28 hours) and three (3) administrative sessions per week (12 hours). Administrative time is allotted to allow the provider ample time to coordinate care and interface with other team members. * Develop and maintain a limited primary care panel focused on diagnosis, management, and treatment of medical ailments as well as wellness and disease prevention. * Assist with providing comprehensive and episodic care to adult patients whose primary care provider is a faculty member in general internal medicine. This includes reviewing the patient chart, taking a history, completing a physical assessment and interpreting diagnostic and radiologic testing along with communicating those findings to the PCP. * Perform preoperative evaluations for the practice. * Proactive patient communication, education and coordination of care between visits and schedule patient appointments for telehealth or in-person visits when appropriate. * Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization. * Counsel and teaches patients and family members concerning disease process and expected outcomes. * Make referrals to other specialists as indicated by the patients' condition. * Document clinic notes and other pertinent data into EMR that is necessary in the care of the patient. Provide information to referring physicians. * Participate in the clinic's weekly high risk rounds and provide updates and insights into patients on panel * Collaborate with clinic leadership to identify in-need patients. * Provide regular feedback to clinic leadership on PCP and NP collaboration, assisting to make workflow changes or provide education on program where appropriate. Minimum Qualifications: Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Additional experience may substitute for master's degree.Three (3) years of related clinical NP experience preferred but not required. Current American Red Cross or American Heart Association CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date. JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis. Licensure and Certifications: Successful completion of the national certification examinations. Current CPR certification required. Maryland CDS and DEA licensure required. Familiarity with regulations and standards governing NPs in the State of Maryland. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date. Additional Knowledge, Skills, and Abilities: * Operates personal computer to access email, computerized scheduling and documentation databases, and other basic office support software. Strong oral and written communication skills required. * Demonstrates ability for self-direction and initiation. Demonstrates ability to work independently as well as collaboratively within team structure. * Demonstrates customer service skills. * Must be able to prioritize responsibilities and work in a fast-paced environment. Technical Qualifications or Specialized Certifications: Completes all required HIPAA and other specified online training courses. Successfully completes all necessary EPIC training courses and other applicable clinical software systems. Physical Requirements: Sitting, standing and walking for extended period. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s). Ability to move standard equipment through a hospital or clinical environment. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
