Job Announcement

Delivers services to individuals who are diagnosed with chronic and persistent mental illness (i.e. Schizophrenia, Bipolar Disorder, etc.) and are receiving community-based services from the Assertive Community Treatment (ACT) team. ACT services are provided in partnership with individuals and their natural supports, in a variety of community settings with a strong emphasis on person-centered planning, strength-based approach to recovery, empowerment, self-determination, and independence.

ACT is a multidisciplinary team made up of specialists (peer support, vocational, and co-occurring disorder), registered nurses, psychiatrists, generalists (clinical case managers), supervisors, and a dedicated administrative assistant. The team is based out of the Gartlan Center in Alexandria, Virginia and operates year-round offering daytime office hours from 8:30 am to 8:30 pm. Monday through Friday and 8:30 am to 5:00 pm on weekends and holidays. Overnight hours are covered through the CSB's Emergency Services in collaboration with on-call ACT staff.

Works alongside clinical staff ensuring that individuals’ medical and behavioral health needs are met. Responsible for securing and storing medications in the PACT medication clinic as set forth in the CSB procedures to meet the requirements of the Virginia Board of Pharmacy. This includes the physical requirements of organization, monitoring temperature, safety, and security of all medications and supplies in the medication clinic for ACT clients. Also responsible for taking vital signs, glucose monitoring, administering intramuscular injections, preparing and dispensing medications, and using a computer to read and enter data into clients’ electronic health records. Incumbent must be able to function autonomously and as part of a team in a fast-paced work environment. The majority of the team’s services occur in community settings such as clients’ homes, hospitals, crisis stabilization units, jails, shelters, libraries, anywhere in the community an individual wishes to be seen, or wherever outreach is required to deliver services.

This position will include a sign-on bonus amount of $2,000 for full-time and $1,000 for part-time positions.

Illustrative Duties

  • Provides nursing services to individuals and/or families in clinics, ambulatory care centers, offices, or community settings;
  • Administers medications and performs treatments;
  • Performs appropriate height, weight, and head circumference measurements;
  • Performs Snellen (eye chart), Ishihara vision (color), and audio(hearing) testing;
  • Administers immunizations under the supervision of a licensed medical practitioner or registered professional nurse;
  • Solicits contraindications and medical history prior to each injection;
  • Assists in pharmacy organization;
  • Performs necessary laboratory tests and/or specimen collections per agency protocol (e.g., venipuncture; finger, foot, and heel stick; blood pressure checks; and urine testing);
  • Records and reports testing and specimen collection findings;
  • Prepares lab specimens collected for shipment to other labs;
  • Maintains a neat clinic area, assuring supply room is properly stocked and organized and that equipment is cleaned, sterilized and maintained in good working order.


Required Knowledge Skills and Abilities

  • Knowledge of practical nursing skills gained through an accredited practical nursing program;
  • Skill in performing technical nursing procedures (e.g., injections, venipuncture, blood pressure screening);
  • Ability to identify questionable test findings;
  • Ability to perform CPR or perform emergency treatment for life-threatening conditions;
  • Ability to elicit sufficient pertinent medical information from a client and record a medical history that is complete and accurate;
  • Ability to establish and maintain good working relationships with clients and staff members.


Employment Standards

MINIMUM QUALIFICATIONS:

Graduation from an accredited Practical Nursing program; plus two years of experience as a Licensed Practical Nurse.

CERTIFICATES AND LICENSES REQUIRED:
  • Possession of current license or a multistate licensure privilege to practice as a Licensed Practical Nurse (LPN) in the Commonwealth of Virginia;
Some positions within this classification will require additional certifications and licenses, as defined by the position description and posted in job announcements.

NECESSARY SPECIAL REQUIREMENT:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.

This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies).

PREFERRED QUALIFICATIONS:
  • Three or more years' experience providing services to seriously mentally ill individuals.
  • Bilingual (Spanish/English);
  • Current certification as a Qualified Mental Health Provider-Adult (QMHP-A);
  • Experience with and knowledge of the Assertive Community Treatment model.
  • Experience working in non-traditional settings (community, jails, shelters, hospitals, etc.).

PHYSICAL REQUIREMENTS:
Ability to stand, stoop, bend, stretch, walk, climb, sit, kneel, crouch, reach, crawl, lift and handle materials with manual dexterity. Ability to lift up to 20 lbs, stand for 2 hours at a time, discern color and take vital signs (blood pressure, weight, abdominal girth, temperature, pulse, blood sugar finger sticks). Ability to physically organize, sort, store, deliver medications and administer intramuscular injections; read and record medication orders and laboratory exam results; and communicate with individuals with special needs in the areas of mental health, developmental disabilities, SA disorders, and primary care. Ability to input, access, and retrieve information from a computer. All duties performed with or without reasonable accommodations.
