The Post-Acute Care Center seeks to hire an experienced Intake Liaison (LPN/RN) for our Post-Acute Care Services Department who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing. The Post-Acute Care Center serves both Home Health and Acute Rehabilitation patients. The Liaison is responsible for processing Home Health referrals.

As a PAC Intake Liaison you will:
• Perform duties and activities related to the clinical assessment of patients referred for home health services.
• Interface with referring facilities, physicians, and staff, in order to efficiently complete the
transition to home health.
• Hold expertise in: referral processes and electronic systems required for completing the job;
educating patients and families on the benefits of AHC's Post-Acute Services; in screening
patients for eligibility for PAC.

Qualifications include:
• Bachelor's Degree required
• Active Maryland license as Registered Nurse (RN) or Licensed Practical Nurse (LPN) required
• 3+ years clinical experience in hospital or post-acute setting
• Preferred experience as a Clinical Liaison or Case Manager
• Self-motivated with excellent time management skills; and be able to work both collaboratively and independently.

Work Schedule:

Variable part-time coverage and scheduled as needed; day time hours and possible holidays.

Candidate must be flexible to work weekends as needed.
