White Oak Medical Center seeks to hire an experienced Outpatient Care Navigator RN who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Outpatient Care Navigator (Case Manager) RN, you will:
•Identify, assess, plan, implement, coordinate, monitor, and evaluate healthcare services to meet an individual patient's specific health care needs in a cost-effective manner. (may include but not limited to home health assessment, telehealth initiation, disease management education, coordinate of post-acute services)
•Complete required documentation in a timely and accurate manner
•Collaborate with other key Care Team roles
•Coordinate patient care with patient, family, staff, physicians, and ancillary/community services to promote quality and cost-efficient care for assigned caseload
•Engage physician practices to review gaps in care for patient population and conduct outreach to facilitate closing the gap
•Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals
•Conduct patient home assessment visit when indicated

Qualifications Include:
•Maryland RN licensure
•Bachelor of Science in nursing or RN enrolled in Bachelor of Science in nursing program with plans to complete within two years
•Active American Heart Association Basic Life Support (BLS) certification required
•3+years' experience in acute care setting
•Prefer at least 1 year of prior experience as case manager, discharge planner or care coordinator
•Empathic toward individual's complex health needs and their families
•Strong communication and customer service skills
•Knowledge of community resources
•Strong writing skills
•Strong organizational skills; able to develop workflows to create more efficient processes
•Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
•Excellent time management skills
•Ability to problem solve and exercise good judgment
