Maryland Care Management Inc. (MCMI) is responsible for managing Maryland Physician Care?s (MPC) statewide provider network of hospitals and physicians. Maryland Physicians Care has been providing services to the HealthChoice Medicaid populations since 2006 and is proud to be celebrating its 25th anniversary. With over 230,000 members, MPC consistently has been one of the third largest managed care organizations in MD. Under the direction of the Director, Quality Management, this position is responsible for coordinating the Plan?s prevention and wellness activities specifically designed to promote the importance and benefits of accessing timely preventive and well care through targeted member/provider education and outreach initiatives. Additionally, this position may include community outreach and public relations initiatives as assigned. The incumbent is responsible for monitoring the Plan?s overall performance against state and federal standards for health education and preventive care and recommending options for improving performance. Develop and monitor the Prevention and Wellness Plan and Work Plan annually. Complete a Prevention and Wellness Assessment including enrollee participation and program impact. Develop and monitor the Outreach Plan and Work Plan. Develop, implement, and ensure ongoing coordination of designated prevention, wellness and outreach activities outlined within the quality improvement plan, Prevention and Wellness Work Plan and Outreach Work Plan. Assists in development of strategies for the Prevention and Wellness initiatives and goals. Assist in meeting Prevention and Wellness budget, as applicable. Coordinate the production of regularly scheduled member and provider newsletters and assists with writing and reviewing of articles as applicable. Coordinate prevention, wellness, and outreach articles on social media. Serve as health plan resource for activities concerning state or federal programs for targeted preventive, wellness, and outreach services. Function as a liaison for the health plan?s approved projects/programs related to outreach efforts with government agencies, community organizations, and public relations initiatives that emphasize health promotion and disease prevention. Collaborate with various health management team members to develop specific interventions that will improve member?s health status, members? adherence to care plan and member?s health literary. Maintain a resource library of educational programs and develop materials and tool for programs sponsored by plan, vendors, equity owner health systems, local health departments and community advocacy groups. Obtain appropriate levels of approval for materials to be distributed to members and providers. Serve as corporate and local resource for reviewing member communication materials for appropriate level of comprehension and overall health literacy, web page content, and obtaining state review and approval as assigned or required. Collaborates with the Community Engagement Department on member outreach projects to ensure P&W and Outreach events are scheduled and conducted. Develop educational materials for members. Assist with training or education of providers. Attend Community Advisory Board meetings. Develop surveys to evaluate and track prevention, wellness, and outreach program for both members and providers. Education and Work Experience: Bachelor?s degree in health-related field (Health Education Degree preferred.) Minimum 3 years computer, health care marketing, health education, outreach experience. Excellent communication skills, public speaking, creative written and graphic arts experience helpful. Demonstrated experience in a managed health care field preferred. At least one year of experience working with local health departments, community agencies or managed care organizations serving Medicaid recipients. Direct Reports: None
