The individual must demonstrate knowledge of the principles of growth and development over the life span, possesses the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.ESSENTIAL DUTIES AND RESPONSIBILITIES:Productivity and Coding Responsibilities Analyzes and evaluates medical records and assigns appropriate ICD-10-CM and CPT diagnostic and/or procedure codes on the medical records in accordance with coding guidelines. Post charges for services provided into hospitals computer information data system. Enter required data into hospitals computer/data information system. Maintains a 97% Quantum Compliance submission rate for inpatient coding. Advises medical staff of appropriate documentation required for coding diagnosis or procedures and make request for changes and/or additional documentation in the medical record when needed. Maintains a coding turnaround time that meets organizational goals. Document appropriate adjustments to coding according to established department procedure. Accuracy of Work Code inpatient and outpatient records with 97% accuracy rate. Consults with medical staff when necessary to secure sufficient information to clarify data for proper coding, and resolve discrepancies in the documentation of information contained in the medical record. Abstract and retrieves medical information verifies the accuracy of keyed data and corrects errors, as necessary. Quality checks work on daily basis Participates in quality improvement initiatives. Researches, analyzes and justifies coding changes/revisions recommended by regulatory groups. Other Required Duties Complete and report daily and submit weekly productivity and time sheets as needed. Assures equipment and IT issues are reported promptly to both IT and supervisor. Alerts and advises supervisor of changes in coding or complications within the medical records.
