Summary

The Senior Social Worker works for the Central Virginia VA Health Care System's Hospital and Clinics in Richmond, Virginia. The Senior Social Worker is part of Social Work Service, and is assigned to the Palliative Consult Team under Geriatrics & Extended Care. The primary purpose of this position is to provide clinical social work services to patients as a part of the Palliative Care Consult Team and serve a patient population with highly complex health and mental health problems.

Responsibilities

The senior social worker is assigned administrative responsibility for clinical program development related to Palliative Care, including coordination of palliative care consults and hospice referrals, and is accountable for clinical program effectiveness and modification of service patterns. The senior social worker fulfills leadership roles on committees as assigned.
Major duties include:
  • Performs specialized treatment of complex physical and mental illness for patients with advanced stage illness or serious illness;
  • Serves on Interdisciplinary Teams, working with patients dealing with end of life issues, terminal or chronic illness and diagnoses;
  • Provides a consultative service to areas of the health care system seeking input from palliative care specialists;
  • Provides counseling and assistance with Advance Directives for Healthcare and end of life planning;
  • Provides education, referrals and/or coordinates required assistance and palliative care services within the VA or in the community;
  • Makes psychosocial and psychiatric diagnoses within approved clinical privileges and scope of practice;
  • Adheres to required documentation standards and procedures;
  • Provides advanced level case management interventions;
  • Provides individualized care coordination and discharge planning;
  • Provides bereavement counseling and support services;
  • Participates in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service, and at the facility level;
  • Develops and implements methods for measuring effectiveness of social work practice and services in the Palliative Care specialty area;
  • Performs other duties within scope of practice as assigned.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. (NOTE: This applies to the past, current, and upcoming VA Health Professional Trainees (HPT) only. HPT is an individual appointed to temporary positions in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. HPTs can be either VA-paid or Without Compensation (WOC).)

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Requires master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless grandfathered by the state in which licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and must be able to provide supervision for licensure.   • English Language Proficiency: Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Preferred Experience: Social Worker experience in hospice or palliative care is highly preferred. Medical social worker experience also preferred.

Grade Determinations: GS-12 EXPERIENCE: Requires at least two years of experience post advanced practice clinical licensure and in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

Physical Requirements: The incumbent must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The incumbent must be able to perform primarily light and sedentary duties with occasionally moderate physical demands with reasonable accommodation if necessary, without endangering the health and safety of the Social Worker or others. Position requires walking throughout the Medical Center, climbing stairs (in the event elevator is not available), standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. Position may require minimal travel into the community to meet with community agencies and partners who aid, help, and provide support to the Palliative Care Team, its Veterans, and caregivers. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
