Services are also provided to individuals in their own homes. The programs honor the dignity of the individuals they serve and are respectful of personal choices. Services are person-centered, strength-based, and recovery-oriented, and are provided at a frequency and intensity that meets each resident's individual needs. The programs, which are nationally accredited and designed to facilitate independence and community engagement, use leading-edge and evidence-based practices that take into account each person's strength, needs, desires, health and safety.We are seeking compassionate people to provide services, supports, and interventions that assist individuals in living in the community and achieving their personal goals. If this is a mission you feel drawn to, we would love to have you as part of our team!Responsibilities include (but are not limited to):Care Coordination and Resource ManagementMoney Management PlanningMedication Monitoring and teaching independenceAssisting clients with grocery shoppingMeal preparation and healthy diet planningEducation on the importance of cleaning routines and self-care skillsSocialization and community integrationTransportationAdvocacyDirect care employees work as part of collaborative service teams that develop and implement individual service plans based on the assessed needs of the individuals served.
