Responsibilities

The general responsibility of the Non-Supervisory Orthotist-Prosthetist is to provide the most complex clinical and laboratory prosthetic and orthotic services to disabled patients. Included in this series is such work as:
(1) Treatment of all amputation levels including upper limb, hip disarticulation levels.
(2) Serving as an advisor to physicians, therapists, and other medical professionals with regard to indications and contraindications of prosthetic and orthotic componentry, prescription criteria.
(3) Authorizing the purchase, fabrication, or repair of prosthetic devices;
(4) Maintaining continuous liaison with manufacturers and dealers of prosthetic supplies, inspecting their facilities and services and participating in the award of prosthetic appliance contracts;
(5) Participating in investigative/research project studies by selecting patients as pilot wearers of newly developed prosthetic items, evaluating the usefulness of such items, and compiling of data; and
(6) Giving technical and administrative advice and assistance on prosthetic problems to other facilities. This work requires a specialized knowledge of the medical and psychological problems directly related to the use of prosthetic devices; and a specialized knowledge and understanding of the fabrication and satisfactory fitting of prosthetic devices. An Orthotist Prosthetist at this level must have analytical ability, social awareness, tact and diplomacy, and good communication skills. The Orthotist Prosthetist at this level must be able to anticipate the need for action. He/she must also possess the ability to deal effectively with individuals and groups and to adjust to difficult or stressful situations.

Duties include but are not limited to:
1. Follows established regulations and procedures to insure patient privacy.
2. Identifies biomechanical condition.
3. Conducts gait analysis, range of motion, coordination and balance, motor skills, and clinical assessment studies in accordance with occupational principles for unusual conditions, multiple complex interrelated conditions, etc.
4. Interviews patients to determine expectations, lifestyle, home/work environment, ability to use the device.
5. Takes necessary measurements, casts or scans to develop positive molds of the affected area of the body, which are used to create the orthosis and/or prosthesis.
6. Recognizes physical abnormalities, deviations, and complicating conditions with potentially life threatening implications.
7. Assesses patient for indications/contra-indications o f appropriate treatment protocols and discusses the assessment with the physician/requesting provider for purposes of developing the overall treatment plan. Has considerable influence in the development of the plan.
8. Considers new and emerging technologies in designing unique or innovative devices to accommodate or treat a wide range of complex medical conditions, which include unusual problems or complications.
9. Considering the information developed through the assessment as well as time management workload considerations, designs custom devices using standard components to optimize patient performance for difficult and complex conditions; may include CAD/CAM systems.
10. Selects materials and components to be used in fabrication of the device and determines appropriate equipment to be used in fabrication of the device or minor modification to orthotic devices and or goods.
11. Fabricates components, or selects and modifies components, and assembles these into the finished device and or works with commercial vendors on fabrication orders. Working knowledge of CAD/CAM system as appropriate in this process and can performs preliminary alignment.
12. Fits the device on the patient, assures preliminary alignment, and performs subsequent dynamic alignment.
13. Assesses and/or modifies fit and function of the device as fabricated and makes appropriate modifications to insure optimum patient outcome.
14. Evaluates patient's ability to don/doff the device, provides training to the patient and/or caregiver in its use and maintenance. This may also include training the clinical staff.
15. Explains procedures and processes to the patient to reduce anxiety and gain support and cooperation in all stages of the process.
16. Maintains the patient's record, including the electronic record, in accordance with established Joint Commission Standards, ABC and or BOC best practice recommendations, VA Medical Center and VISN policies.
17. Attends professional and manufacturer's training seminars.
18. Completes mandatory training per hospital requirements.
19. Maintains professional credentials and/or competency.
20. Observes appropriate, established infection control guidelines.

BASIC REQUIREMENTS
Citizenship. Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
Education
(1) A bachelor's degree or higher in orthotics and prosthetics from a Commission on Accreditation of Allied Health Education Programs (CAAHEP) accredited program .
OR,
(2) A bachelor's degree or higher in any major along with a post-graduate orthotics or prosthetics certificate from a CAAHEP accredited program .
Foreign Graduates. Graduates of foreign degree programs must have proof of a minimum of a Bachelor's degree from an accredited college or university (or foreign equivalent, as verified through an independent credential evaluation company), with a specialization in orthotics and prosthetics.
Board Certification. Candidates must be board certified by the American Board for Certification in Orthotics and Prosthetics (ABC) or the Board for Orthotist/Prosthetist Certification (BOC) as an orthotist, prosthetist, or orthotist/prosthetist. The board certificate must be current and the applicant must abide by the certifying body's requirements for continuing education. Certification as a technician, pedorthist, assistant or fitter does not meet this requirement.
Loss of Credential. An employee in this occupation who fails to maintain the required certification must be removed from the occupation which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation for which he/she qualifies if a placement opportunity exists.
Grandfathering Provision. Orthotists, prosthetists, or orthotist/prosthetists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held , including positive education and certification that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
(1) Orthotists, prosthetists, or orthotist/prosthetists that do not meet the basic requirements for education and certification may be reassigned, promoted up to and including the full performance (journey) level, or demoted within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
(2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard.
(3) Employees initially grandfathered into this occupation who subsequently obtain additional education and/or certification that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
(4) If an employee who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
Physical Requirements. Daily intermittent walking, standing, bending, stooping, reaching, and carrying of lightweight items required. Rigid deadlines and need for high degree of accuracy periodically becomes stressful.
English Language Proficiency. Orthotists, prosthetists, or orthotist/prosthetists must be proficient in spoken and written as required by 38 U.S.C. 7402(d).

Experience. Completion of 1 year of experience as an orthotist/prosthetist equivalent to the GS09 grade level that demonstrates possession of the knowledge, skills, and abilities needed to provide orthotic/prosthetic services in a hospital setting.
Demonstrated Knowledge, Skills, and Abilities. You must demonstrate the following KSAs:
1 . Ability to attend clinics and participate as a member of the treatment team. This includes knowledge of medical terminology, anatomy, physiology, biomechanics, kinesiology, physics, and etiology of diseases as well as knowledge of psychology and age-related competencies.
2 . Knowledge of the Healthcare Common Procedure Coding System (HCPCS).
3 . Ability to conduct clinical patient analyses such as gait, range of motion, life style, etc., for patients with complex but typical conditions.
4 . Ability to take necessary measurements, casts, or scans to develop positive molds of the affected area of the body to create orthoses and/or prostheses.

Performing a broad range of orthotic and prosthetic services designing custom devices using standard components to optimize patient performance for difficult and complex but typical conditions. Assignments included clinic attendance to provide information on standard and stock items which are readily available. The work involved identification of the biomechanical condition; explaining procedures to patients to reduce anxiety and gain support and cooperation; detailed patient assessment including conducting gait analysis, range of motion, coordination and balance, and motor skills; interviewing patients for lifestyle assessments; taking necessary measurements, casts, or scans to develop positive molds; recognizing physical abnormalities, deviations, and complicating conditions; fabricating, fitting, and evaluating these custom devices; and maintenance of the patient's record. The work may have included use of Computer Assisted Design/Computer Assisted Manufacturing (CAD/CAM) systems.

Grade Determinations: GS11
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, you must demonstrate the following:
