As a Case Manager /Care Navigator you will:
•Complete comprehensive psychosocial assessments and review clinical needs of patients as they transition to the next level of care
•Collaborate with physicians, social workers and others to foster a coordinated approach to discharge planning and prevent delays in patient care
•Communicate barriers to leadership for resolution and trending
•Identify services and connect patients to resources available in the community
•Participate in Interdisciplinary Rounds and other patient care conferences
•Collaborate with Utilization Review staff
•Review medical records for clinical criteria, appropriate level of care and compliance with standards

Qualifications Include:
•Bachelor of Science in Nursing or RN enrolled in Bachelor of Science in nursing program with plans to complete within two years
•Maryland Registered Nurse license
•1+ years of experience in patient navigation, discharge planning, or care coordination
•Time Management Efficient and accurate reviews
•Knowledge of disease management
•Prior experience with Cerner, EMR and All Scripts is preferred
•Knowledge of Interqual/MCG preferred
