Essential Job Functions: 1. Performs the initial intake of patients being referred to hospice program and follows up with all necessary contacts with patient, family, physician, insurance representative and other hospice staff. 2. Performs the assessment function for all clients and derives nursing diagnoses based on assessment data. 3. Maintains timely and thorough documentation on all contacts regarding each referral, intake and assessment. 4. Acts as liaison nurse with WMC, monitoring in-patients to ensure appropriate care and treatments are authorized by BRH while a bed transition at WMC. 5. Develops a plan in conjunction with supervisor to visit with individual physicians to promote the use of hospice services with appropriate patients. 6. Serves as a referral source for all incoming questions from individuals contacting hospice. 7. Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems and prevent potential problems. 8. Acts as a patient advocate. 9. Implements nursing interventions to relieve pain and other symptoms of life-limiting illnesses. Delivers care within the scope of nursing practice and in accordance with accepted standards of practice. 10. Practices universal precaution in all contacts with patients and special isolation techniques as warranted by patient diagnosis. 11. Teaches and demonstrates care to be provided by the patient, family or home health aide as warranted. Provides ongoing supervision and teaching as warranted. 12. Provides emotional support to patients and families in a manner that fosters independence, affirms existing and new strengths and promotes and develops strengths. 13. Identifies need for intervention by other members of the interdisciplinary team on an ongoing basis; collaborates in the planning of such care. Provides complete and accurate supporting documentation. 14. Provides for continuity of service across care setting through effective information management, including both verbal and written communication. 15. Participates in the orientation of new members of the interdisciplinary team. 16. Teaches Hospice philosophy and techniques of pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted-living facilities, etc.). 17. Respect the autonomy and right to confidentiality of all patients and families. 18. Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback. 19. Recognizes elements of patient and family systems that are not amenable to change. Evaluates own effectiveness in establishing and maintaining therapeutic relationships with patients and seeks assistance of supervisor or other resource persons in responding to opportunities for improvement. 20. Acts independently and responsibly to perform job duties on a consistent basis. 21. Is sensitive to the stressors experienced by colleagues in working with Hospice patients and families. 22. Carries out the mission and values of Blue Ridge Hospice. 23. Admits and oversees/Case Manager of all bed transitions at WMC. 24. Proficient in EMR to include Epic. 25. Shares weekend and holiday rotation with the Intake Team. 26. Must be a licensed driver with an automobile that is insured in accordance to state or organization requirements and is in good working order. Must be willing to drive the entire service area. 27. Other duties as assigned. Job Requirements: Education and Experience: 1. Graduate of an NLN accredited nursing program, Bachelor of Science in Nursing preferred. 2. One year of medical-surgical or oncology nursing experience required 3. Minimum of six months hospice home care clinical experience preferred. 4. Current unrestricted Virginia license to practice as a registered nurse or willing to obtain Virginia license prior to employment. 5. Certified in hospice nursing preferred.
