Russell Tobin is looking for a Lead Care Coach in Baltimore, MD for a contract role of 6 months (possible contract to hire!) with one of our clients.Shift: M - F/ 9am to 5pm, Remote till COVID.Duties:The Lead Caregiver Coach / Care Coach provides broad wellness navigation and support, as well as non-clinical coordination of activities for caregivers that are entrusted in the care of a loved one.The Caregiver Coach serves as a trusted and supportive resource for caregivers, providing guidance, encouragement, and resources to assist them along their caregiving journey.The Care Coach sources and recommends non-clinical, social, and other resources across the community, knowledge banks, etc. to help caregivers care for their loved ones and maintain their own wellbeing.This role will be an active participant in the ideation/creation/designing of new platform capabilities that are needed in caregiver support, leveraging their experiences and subject-matter expertise as part of the product creation and implementation lifecycle.This role includes caregiver planning, coaching, regular telephonic and video dialogue, providing social service-like referrals to key sources of need.This role is at the center of the platform, ensuring caregivers have a trusted expert to support them in their specific journey.The Care Coach will take part and be fully engaged in the team direction, address problems proactively and provide subject matter input to support the team in meeting its objectives.Principal Accountabilities:Under the general direction of a Supervisor, the Caregiver Coach / Care Coach accountabilities include, but are not limited to, the following:Manages a roster of 30 caregivers.Engages with assigned caregivers, ensuring that assessments of caregiver needs are performed, documented, evolved as needs change, and tasked.Creates a Caregiver Plan with the caregiver to ensure the needs of caregivers are identified and a plan of action is documented.Proactively contributes as a value-add member of the product ideation, creation, and design efforts to create the best in breed caregiver support platform.Proactively ideates with team members to bring subject matter expertise to the product design.Provides suggestive and supportive outreach to all community sources of referrals for follow-up on social, wellness, and other caregiver needs.Collaborates with the caregiver to address those needsConducts needs assessments which include, but are not limited to, psycho-social, physical, behavioral, environmental, and financial parameters.Develops, document, and implements a caregiver plan, providing appropriate resources to address social, physical, mental, emotional, spiritual and supportive needs.Communicates the caregiver plan of support and serves as a point of contact to answer questions in capturing value from those support services.Implements, coordinates, and monitors strategies for caregivers to support the caregiver quality of life.Assesses and reviews caregiver plan regularly to identify and address gaps; works with caregivers to adjust the plan, when necessary.Acts as an advocate for caregivers needs.Communicates with caregivers through the use of technology (online, telephonic, video).Performs ongoing monitoring of the caregiver plan to evaluate effectiveness and helps measure the effectiveness of interventions as identified in the caregiving plan.Serves as a communication channel for caregivers; acts as a source of information and provides coaching guidance.Acts as resource to the caregiver to provide information regarding community support organizations, social services, and other support structures which would benefit the caregiver.Educates other team members, supporting staff, caregivers and families regarding the caregiver journey and strategies with a focus on wellness.Facilitates a team approach to ensure appropriate engagement and effective delivery of quality services across the continuum.Maintains professional relationships with stakeholders.Assists with orientation and mentoring new team members as appropriate.Other duties as assigned.Scope Data:Incumbent is responsible for ensuring the appropriate caregiver support activities and services are provided for the targeted population.The Caregiver Coach / Care Coach must be well versed in all aspects of the program.Ability to interface with caregivers and other team members through technology.Required Education/Experience/Skills/Abilities:Bachelor's Degree requiredMaster's Degree, Social Work (MSW) work beneficialLSW, LMSW, LCSW or recognized jurisdictional credential(s) preferredCertified Case Manager designation welcomeThree to Five Years of experience in social work, case management, caregiver management, homecare, hospice, or managed careSolid comfort and with web based, telephonic, and video technology Interested!
