Job DescriptionWe Are HiringNot remote: Onsite locationA' This position will work full-time; Monday through Friday 8:00 am to 4:30 pm You will support our patients within our primary care medical group settings. You will utilize your critical thinking skills, clinical experience and case management techniques to support our patients with various social determinants of health. Your work will directly impact the ability of our patients to stay well and to help build a health community so that our patients can achieve an optimal level of health.A' What You Will DoCoordinate the overall interdisciplinary plan of care for patient, from admission to discharge.Create plan for care across the continuum, integrating patient/family preferences and values.Monitor patient care through assessments, evaluations and/or patient records.Advocate for resources and removal of barriers.Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.What You Will NeedLicensure / Certification / Registration:Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.BLS Provider credentialed from the American Heart Association (AHA) preferred.Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.Education:Diploma from an accredited school/college of nursing required.BSN preferredminimum 1 year care management experience preferredCommunity Health Experience also a plusAdditional PreferencesWhy Join Our TeamServing the greater Baltimore area, Ascension Saint Agnes is recognized as an employer that provides an open, highly collaborative work environment where our workforce reflects the diversity of those we serve.
