Summary

The Intensive Care Unit Clinical Pharmacist Specialist is responsible for planning, implementing and operating the critical care drug monitoring and utilization programs primarily for the medical/surgical intensive care units. He/she is the intensive care unit expert in all areas related to critical care agents and functions as a consultant to other health care professionals.

Responsibilities

The incumbent is the specialist involved in clinical investigations and serves as an expert in the study information on chemistry, pharmacokinetics, toxicology, drug interactions, etc. He/she will also be responsible for developing, providing, and supporting the educational programs related to critical care for Pharmacy.

The incumbent will be responsible for the development of highly innovative patient care activities in the delivery of health care for the medical/surgical intensive care units. Participate in daily multidisciplinary team rounds with the medical/surgical intensive care unit teams and provide drug information, pharmacokinetic consultation, dose implementation/adjustments, observations on patient response to therapy, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomics.

Interviews and evaluates ICU patients upon admission to the hospital and as necessary during hospitalization for appropriateness of pharmacologic therapy and makes a pharmaceutical care plan for those patients who need pharmaceutical care, and will document the pharmaceutical care plan in the Computerized Patient Record System (CPRS).
  • Develops criteria for utilization of new medications specific to the critical setting (as assigned) for Pharmacy and Therapeutics Committee P&T and Medical Executive Committee MEC approval.
  • Provides consultative services to all professional staff regarding critical care medications.
  • Attends all Code Blue emergencies during the tour of duty.
  • Provides consultative services to all professional critical care staff regarding drug therapy.
  • Provides care and/or services appropriate to the age of the patients being served. Assesses data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs and to provide care needed as described in the services' policies and procedures.
  • Designs and coordinates drug utilization evaluations including the collection and compilation of data, preparation of reports, and presentation to the Pharmacy and Therapeutics Committee (as assigned).
  • Participates in medication reconciliation process: Interviews and evaluates patients upon admission, daily during hospitalization and upon discharge for appropriateness of pharmacologic therapy. Process and/or insures the processing of all discharge medications.
  • The incumbent will possess a high level of independence designated by their Scope of Practice and will be recognized by their peers as performing assignments that require an exceptional level of competence. A broad Scope of Practice will be designed commensurate with the complexity of the clinical pharmacy services provided to the intensive care units.

    Qualifications

    To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 05/07/2021.

    Basic Requirements:
    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
    • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.

    Preferred Experience: The Intensive Care Unit Clinical Pharmacist Specialist must be a licensed pharmacist with a Pharm.D. preferably with an accredited residency in Critical Care (PGY 2), or equivalent experience demonstrating competency in this area. The incumbent is expected to obtain board certification that is recognized by the American Pharmaceutical Association's Board of Pharmaceutical Specialties, or equivalent.

    Grade Determination GS-13:

    (a) Experience. In addition to the basic requirements, candidates must meet one of the following:

    1. 1 year of experience equivalent to the next lower grade level (GS-12)Pharmacists assigned to this position must demonstrate the following KSAs:

    a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
    b. Expert understanding of regulatory and quality standards for their program area.
    c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
    d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
    e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

    Physical Requirements: Physical demands of the position require regular and recurring physical exertion, involving standing for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing up to 40 pounds, pushing and pulling of large carts. The job may include significant time at a computer terminal.
