White Oak Medical Center, brand new state of the art facility, seeks to hire an enthusiastic and goal oriented person as the Inpatient Care Navigator (Case Manager / Discharge Planner) Manager. The right person will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.

As the Inpatient Care Navigator Manager, you will:

•Support departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate
•Lead team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner
•Ensure active participation in Interdisciplinary rounds and other patient care conferences
•Lead delivery of care provided by Care Navigation team to ensure that patients' needs are addressed, and that documentation meets standards
•Implement the departmental strategic plan
•Collaborate with Utilization Review leadership to understand trends of discharge barriers and denials
•Facilitate the development of systems, processes and PI that drives optimal performance for the department
•Oversee and evaluate the performance of the Care Navigation team
•Mentor and transform the performance of groups in a manner consistent with the organization's mission and values
•Demonstrate excellent collaboration with both internal and external customers and stakeholders
•Maintain a climate that attracts, retains and motivates staff

Required Qualifications:

•Bachelor of Science in nursing required, Master's degree preferred
•1-3+ years' experience in patient navigation, discharge planning, or care coordination
•At least one year of prior leadership experience required
•Maryland RN licensure
