The Care Navigation Manager is responsible for ensuring collaboration with patients and their care team so that patients are navigated through the continuum of care. The manager will role model professionalism and exemplary standards of practice while mentoring, evaluation, and facilitating the professional growth of their team.

Work Schedule:

Principle Duties and Responsibilities
  • Supports departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate
  • Leads team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner
  • Ensures active participation in Interdisciplinary Rounds and other patient care conferences
  • Leads delivery of care provided by Care Navigation team to ensure that patients' needs are addressed, and that documentation meets standards
  • Collaborates with Utilization Review leadership to understand trends of discharge barriers and denials
  • Facilitates the development of systems, processes and PI that drives optimal performance for the department.
  • Oversees and evaluates the performance of the Care Navigation team

Required Qualifications:
  • Able to mentor and transform the performance of groups in a manner consistent with the organization's mission and values.
  • Demonstrates excellent collaboration and with both internal and external customers and stakeholders; able to maintain and manage customer relationships
  • Maintains a climate that attracts, retains and motivates staff
  • Bachelor of Science in nursing required, Master's degree preferred
  • 3 - 5 years' experience in patient navigation, discharge planning, or care coordination
  • At least one year of prior leadership experience required
  • Maryland RN licensure
  • Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC
