This Clinical Program Manager is responsible for the professional and administrative management of an assigned area in pharmacy service, plays a defined role in budgetary execution, and serves as a consultant and midlevel provider who functions to initiate, modify or discontinue medication therapy. Extensive documentation and clinical progress note writing are required of this position. S/he may be involved in research activities and precepting of pharmacy trainees.

The major duties of the Clinical Pharmacy Program Manager, include but are not limited to, the following:
  • Manages the activities of assigned Clinical Pharmacy Specialist and has the oversight of ongoing clinical pharmacy services. Assures that the pharmaceutical care needs of patients are met across the continuum of care within the scope of their clinical specialty area.
  • Ensures implementation of performance standards, assures that clinical practice is compliant with all regulatory and accrediting body requirements. Develops policies and procedures relative to the assigned area of practice.
  • Designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities.
  • Interviews prospective candidates for appointments, recommends appointments, advancements, or promotions or disciplinary actions.
  • Maintains direct or indirect oversight of the Pharmacy Residency Program and other educational programs if applicable to his/her area of practice. S/he may serve as a Residency Program Director (RPD) if a residency program is established at their site and if selected for the position of RPD.
  • Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
  • Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor.
  • Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required.
  • Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states specific to his/her area of clinical specialty. including but not limited to - diabetes, hypertension, hyperlipidemia, anticoagulation, COPD, and heart failure issues.
  • Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Monitors patients prospectively for any deviation from the accepted treatment plan, for the efficacy of the plan, and for any adverse reaction or drug interaction occurring during the execution of the plan.
  • Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by peer review.
  • Assists the area of practice in meeting all of The Joint Commission medication management standards by having an expert understanding of regulatory and quality standards for the area of practice.
  • Supervises pharmacy trainees and residents assigned to participate in the program.
  • Demonstrates knowledge of the standards related to Ambulatory Care, Hospital, and Behavioral Health drug use including but not limited to, the standards established by DEA, FDA, VA, the state, and The Joint Commission.
  • Serves as an authority on drug usage, interactions, over dosages, and compliance to medical staff, nursing staff, technicians, aides, students, and pharmacists of lower grade.
  • Keeps abreast of changes in the Pharmacy Service (i.e. formulary additions and deletions, interdepartmental memoranda, relevant station memoranda, and policies and procedures) and maintains a general knowledge of VA, DEA and The Joint Commission regulations.
  • Serves as a clinical pharmacy preceptor for Doctor of Pharmacy students from the appropriate affiliated Schools/Colleges of Pharmacy. Serves as a preceptor for PharmD residents and maintains a sustained record of contribution and commitment to pharmacy , as defined by ASHP.
  • Participates in P&T Committee functions and assignments as required, e.g., ADR monitoring, medication error reporting, medication utilization evaluation activities, drug monographs/reviews, etc.
  • Provides formal education and clinical education in the area of expertise to pharmacists in training.
  • Participates in medication use evaluations activities, audits or quality management monitoring as designated by supervisor.
  • Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects. Assists in preparing for special accreditations and reviews (e.g. The Joint Commission, Inspector General, etc,) and meets with reviewing/inspecting officials and pharmacy management.
  • Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization.

Basic Requirements:

(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)

(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determination: GS-13

(a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.

(b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the KSAs identified in subparagraph 3b(3)(b)1 above.

a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience:
(1) 3 years of supervisory experience.
(2) 3+ years of Clinical Pharmacy Specialist experience managing multiple disease states within a clinic setting.
(3) Completion of an ASHP accredited PGY-1 Pharmacy Residency.

Physical Requirements: The work is primarily sedentary, although physical effort will be required. Some lifting, bending and moving of videoconferencing or technical equipment for installation and troubleshooting is required. Occasionally climbs ladders to adjust overhead equipment and lifts or moves objects weighing up to forty pounds; occasionally lifts or moves heavier items with assistance from others.
