Summary

The GS-5 Nursing Assistant (NA) performs nursing procedures and duties on an assigned patient area and has special responsibilities with his/her assigned patients, using a team nursing approach in providing holistic care. He/she functions independently under the purview of the Nurse Manager/Assistant Unit Manager or designee. He/she will assist supervisory staff by providing assistance and guidance to lower level Nursing Assistants during the tour of duty.

Responsibilities

The NA functions as a highly skilled member of the staff and provides administrative and clinical support for the delivery of acute and non-acute care in the Medical Center.

Major Duties:

- Ability to effectively communicate, using the English language, in writing or orally to provide patient care.
- Stand and maintain balance for the assigned work shift.
- Walk for long periods of time and significant distances no more than six (6) hours continuously.
- Able to bend, stoop, kneel, grab, grasp, climb stairs, and squat while performing patient care activities. He/she must have full range of motion in the involved joints.
- Lift, move, or transfer patients without any personal limitations or physical restrictions.
- Lift or move objects without any personal limitations or physical restrictions.
- Push, pull or carry equipment, supplies, wheelchairs, treatment carts or other devices without any personal limitations or physical restrictions
- Maintain physical tolerance and endurance while providing nursing care for the assigned work shift.
- Perform manual psychomotor skills utilizing fine motor skills.
- Type/keyboard and/or write with pen or pencil.
- Able to hear call lights, emergency alert systems, telephones, pagers, and patient/staff verbalizations.
- Able to see IV drips, medical equipment calibration numbers, 6 point font print and objects of similar size.
- Have olfactory senses intact to identify essential smells relevant to patient care to include bodily fluids, microscopic infestations, and environmental emergencies.
- Have tactile abilities to accurately detect changes in body temperature, changes in skin integrity and physical changes in the patient's body such as lumps, bumps, etc.
- Ability to rotate to any tour of duty as needed.
- Ability to work on any unit and be able to be pulled to another unit at a moment's notice as patient care demands require.
- Ability to work overtime up to four hours.
- Able to enter appropriate data and other documentation into the computer system without delay
- Ability to prepare patient appropriately in assigned clinical setting.

Work Schedule: Rotating Shifts to include days, every other weekend and alternating Holidays.

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education or Training: One year above high school with courses related to the [NA] occupation. Completion of an intensive, specialized, occupation-related course of study of less than one year [ ] as a nursing assistant may also meet in full the experience requirements for GS-3.
  • Physical Requirements: The examination will be concerned with the physical, cognitive and emotional ability of the applicant to perform the essential functions of the proposed assignment satisfactorily. Laboratory and ancillary tests are authorized as required by Federal regulatory agencies and only tests which assist the Employee Occupation Health (EOH) provider in determining whether or not the candidate meets the physical requirements and medical standards of the job may be performed. Reference: VA Directive and Handbook 5019.
  • Grandfathering Provisions: All Nursing Assistants employed in VHA on the effective date of this qualification standard are considered to have met all qualification requirements for the series and grade held that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: 1. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. 2. If an employee who was retained (grandfathered) under this provision leaves the occupation, the employee loses protected status of the grandfathering provision and must meet the full VA qualification standard basic requirements in effect at the time of reentry to the occupation.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Preferred Experience:
  • 2 years acute care experience
  • Knowledge of EKG procedures
  • CNA Certification not required but preferred.

Grade Determinations: Experience. One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs in subparagraph (c) below;

Examples of assignments and experience at the GS-4 level include but are not limited to:
1. Ability to provide a range of patient/resident care and contribute to the completion of the nurse care plan and recovery of patients/residents.
2. Ability to observe patients/residents' or resident's physical or emotional status and refer deviations and/or problems to supervisor in a timely manner.
3. Ability to properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc.
4. Ability to communicate effectively with patients/residents, their families and other health professionals.

OR

Education. Successful completion of a 4-year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree; AND Demonstrated KSAs: 1. Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse. 2. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new CNAs by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting. 3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc. 4. Ability to properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, blood glucose monitoring machine, specimen collection, etc.

Assignment. This is the full performance level for CNAs. CNAs at this grade level function as a member of the nursing care team and assist licensed nursing staff in the care of patients/residents receiving outpatient care, home or community living care or other patients/residents who are in acute, sub acute or chronic states of illness. The distinguishing factor is that patient/resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements. CNAs at this level also assume more of a peer education/mentorship role.

The full performance level of this vacancy is GS-5.

Physical Requirements: Work will require long periods of regular and recurring standing and walking with regular and recurring lifting and positioning of patients while maintaining balance. Must be able to bend, stoop, squat, kneel, move body and extremities quickly, push and pull equipment, and carry equipment and supplies without any personal limitations or physical restrictions. Long periods of sitting with an assigned patient may be required. Physical endurance and physical tolerance is required for a 12 hour shift as assigned. These are essential functions of the position.

The employee is alert to the possibility of patient assault and may need to protect herself/himself using non-abusive physical intervention techniques. The incumbent must have full range of motion of each joint. He/she must be able to grasp, maintain balance, and lower a patient safely to the floor unassisted while physically controlling a violent patient. They must have fine motor skills to effectively operate locking restraints. These are essential functions of the position.
