The Director will lead oversight and implementation of and execution of the Clinical Integration initiatives and the Care Transformation programs in order to contribute toward achievement of physician practice annual goals and long-term Pop Health strategic plans. The Director will oversee initiatives related to enhanced clinical efficiency, successful achievement of payer-focused outcomes and CMS regulatory requirements are achieved. The Director will also mentor and develop staff members and support the overall growth of knowledge and skill within CIN and CTO. The Director of CIN and CTO performs principle duties and responsibilities in a manner consistent with the organization's mission, values and goals

Duties and responsibilities include, but are not limited to:

• Drives and directs the design and execution of the Clinical
Transformation programs to successfully achieve annual goals and
priorities and progress toward long-term strategic plans and growth
department. Ensure appropriate focus of activities and integration with cross-functional stakeholders with seamless execution.
• In conjunction with senior leaders and Medical Director set the strategic and
operational direction of the Department and to identify key priorities. Monitor
KPIs related to initiatives to determine short-term and long-term success
measures. Oversee CTO Board, subcommittees and collaborate with Medical
Director.
• Responsible for value-based outcomes of CTO and CIN; stays at the forefront
of value-based care to gain new ideas and approaches
• Oversees analytics and ensure all assigned programs demonstrate appropriate
synthesis of insights from data, clinical evidence, operational realities, and
payer trends and anticipated evolution.
• Oversees regulatory requirements and reporting for the MDPCP Program
Develop and enhance partnerships with payers and external stakeholders to
accelerate success in total cost of care and payor contracts; Retains oversight
in ensuring payor contract targets are achieved
• Manages department budget
• Drives physician practice engagement and manages relationships with key
stakeholders to achieve payer focused outcomes in patient experience, quality
metrics, and utilization.

Requirements:

• B.A. required
• MBA or MHA required
• 5 plus years of strategy and project management experience,
preferably in a
health care setting
• Health care administration experience preferred
