Job Summary MedStar Health operates several digital platforms that gather information from patients including self-reported data about symptoms, compliance with medical plans and processes and physiologic data such as blood pressure, temperature, blood glucose and others to assist in care. These digital platforms, known as Remote Patient Monitoring (RPM) collect, transmit, store and display this data in clinical monitoring dashboards that the can be used to identify progress or concerning symptoms or data. This position will be responsible for monitoring the home-submitted data from patients and helping to link signals of concern with appropriate care under the supervision of the Connected Care provider team. The time spent monitoring and interacting with patients will be documented to allow billing for RPM services. In addition to RPM activity, the Connected Care specialist will assist the Connected Care provider team with digital services to support patients and providers, including centralized processing of prescription refill requests, responding to lab results, and the scheduling & intake of patients for rapid-access telehealth visits with the Connected Care provider. All roles are performed with provider supervision and back-up.Minimum Qualifications Education/Training High school diploma or GED required.Experience Completion of an accredited medical assistance certification program, additional clinical qualifications would be appropriate in certain uses cases and would be specified in relevant job descriptions.License/Certification/Registration Completion of an accredited medical assistance certification program.Knowledge, Skills & Abilities Ability to communicate clearly, effectively and with courtesy with patients, clinical and administrative team members and physician staff. Fundamental computer skills needed but platform specific training will be provided. Will receive training and must be able to incorporate fundamental aspects of the clinical organization including clinical operational workflows and patient confidentiality. Ability to work independently, especially if engaged in a work from home arrangement, independent ability to maintain network security, technical troubleshooting, connectivity and reliable hardware would be required.Primary Duties and Responsibilities Active review of patient data in an electronic clinical dashboard. Access to and time spent reviewing a patient's data is tracked by platform.Engaging patients by phone, text or possibly video to: Remind patients to complete data entry as needed; Respond to alerts related to patient symptoms or physiologic data (per algorithmic clinical guidance); Respond to patients for minor technical troubleshooting; Respond directly to patient's clinical questions/concerns with scripted and algorithmic responses.Communicate with supervising clinicians regarding concerning findings. Collaborates with the multidisciplinary Connected Care team to develop and coordinate the plan of care.Navigate the patient to other care options as directed by scripted clinical guidance or supervising clinician.Process proposals for medication refill authorization using the digital refill authorization platform and forwards proposals to the provider for signature, under supervision of the Connected Care provider team.Processes normal laboratory results and communicates those results back to the patient using standard scripting under the supervision of the Connected Care provider team.Assists with logistics, scheduling and standard intake procedures for Telehealth visits with the Connected Care provider team in support of a rapid access protocol for patients with urgent issues who cannot be accommodated by their PCP's schedule in an adequately timely manner.Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
