We are currently looking for a Service Coordinator II. Hours: Monday - Friday: 9:00am - 5:00pm Our Senior Communities program develops and operates affordable, supportive communities for older adults with a resolve to nurture a spirit of purpose, wellness, and harmony among both our residents and colleagues.The Service Coordinator II educates, assists and advises residents and families about the resources and services to support living in the apartment setting, encouraging and coaching residents to be proactive self-advocates in meeting their social, psychological, and physical needs. Responsible for the supervision of the Service Coordinator Assistant. Job Responsibilities: Creates partnerships and educates and engages older adults in accessing information, resources and services to support aging in place in the affordable apartment setting Builds one-on-one, long-term trusted relationships Assesses service needs (in such areas as health care, counseling, nutrition, transportation, finance, home management, legal services) Determines eligibility for benefits and services Provides case management Creates mutually beneficial relationships with organizations and services of the local community This position will require the following: Bachelor's degree and three years experience in Human Services, preferably in the field of aging or an equivalent combination of education and experience A valid driver's license with no more than 3 points and reliable transportation to and from assignments We offer a wide range of benefits to meet the ever-changing needs of our employees including but not limited to: Health/Dental/Vision Vacation/sick/holiday pay 403(b) Retirement Plan with a discretionary employer contribution Join Catholic Charities of Baltimore where you will be an important member of an trauma-informed care organization that promotes a culture of safety, empowerment, healing and self-care, and that believes that every person has infinite worth and promise.
