White Oak Medical Center seeks to hire an experienced Inpatient Care Navigator RN who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing. We are currently offering a $5,000 sign-on bonus for qualified candidates!

As the Inpatient Care Navigator, RN you will:
• Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization
• Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers
•Complete comprehensive psychosocial assessment and review clinical needs of patients as they transition to the next level of care
•Collaborate with physicians and social workers to foster a coordinated approach to discharge planning
•Utilize clinical knowledge to participate in treatment plan discussion with clinical teams
•Identify and connect patient to services and resources available to them
•Communicate with Utilization Review staff on any denials, issues or barriers to discharge

Qualifications Include:
•Minimum required- Bachelor of Science in Nursing
•Minimum of 1-3 years nursing experience required
•Licensed Registered Nurse
•Prior case management or discharge planning experience is preferred
•Prior experience with Cerner EMR and All Scripts is preferred
