Summary

The Lead Home Telehealth Care Coordinator Registered Nurse is a professional who provides supervisory oversight and clinical guidance to program staff and is administratively responsible and accountable to the Chief of the Telehealth Service. This RN position is critical to meet the projected workload demands of the day-to-day operations of the Home Telehealth program within the Central Virginia VA Health Care System located at the Richmond, Virginia VA Medical Center Facility.

Responsibilities

The incumbent provides professional assessment, coordination and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are offered, and that progress is being made. The Lead Care Coordinator provides ongoing evaluation of care management services provided by program staff.

The incumbent is a member of a team who uses innovative telecommunication technologies to enhance care coordination and to proactively manage chronic diseases to prevent healthcare crises. The goals of the HT program are to improve chronic disease management, improve patient satisfaction, increase access to services and reduce resource usage thus decreasing health care costs.

The target population for the HT can include Veterans with the following diagnoses:
  • Congestive Heart Failure
  • Chronic Obstructive Pulmonary Disorder
  • Hypertension
  • Diabetes Mellitus
  • Wound care management
  • Chronic Mental Health diagnoses to include Post Traumatic Stress Disorder, Depression, Bipolar Disease and Substance Abuse
  • Obesity
  • Infectious Disease (Hepatitis C or COVID-19 Infection)

Clinical responsibilities include, but are not limited to:
  • Provides initial and ongoing assessment of patients to identify needs issues, care goals and appropriate resources necessary for care management.
  • Provides supervision of program staff involved in the day-to-day clinical operations of care coordination service delivery. This includes assuring accuracy of information provided in the forms of progress notes, reports, presentations, and briefings on program patients and operations.
  • Provides leadership in application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post hospitalization recovery and health maintenance/improvement.
  • Sets clinical care goals, short and long term, in collaboration with patient, provider(s), and family members.
  • Functions as a systems coordinator for the plan of care; monitors progress through the expected hospital course and intervenes as appropriate to facilitate achieving patient outcomes within anticipated timeframes. Coordinates care and discharge planning with the patient's primary care provider and team.
  • Collaborates with patient and care providers in any and all settings where care is being provided to evaluate and update changes in the therapeutic plan of care and patient management.
  • Recognizes complex situations that impact patient care and intervenes, using sound judgment, professional attitude and appropriate channels.
  • Recognizes impact of age-specific care needs and incorporates this into the assessment process. Also, incorporates these age-specific needs into care as reflected by modification of treatment plans.
  • Maintains a working knowledge of resources available at the facility and in the community.
  • Appropriately documents own interventions and oversees appropriate health team documentation of patient care.
  • Assesses patient for use of appropriate telehealth devices, follows procedures for ordering or leasing of equipment, documents installation and education in computerized patient record.
  • Ensures patients are stratified by risk and manages preparation for and activation of Continuity of Operation Plans (COOP) as part of facility, VISN and national emergency operations.

Work Schedule: 8:00am to 4:30pm,Monday through Friday. Management reserves the right to determine and/or change the tour of duty.

Qualifications

    Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Minimum of 3 years successful clinical practice preferred. Demonstrated ability in the area of interpersonal relations, critical thinking, problem solving and conflict resolution.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
    Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.     BLS/ACLS Requirements: Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification in order to avoid any delays in the hiring process.

    Physical Requirements: The work is primarily sedentary, although some physical effort is required. Some lifting, bending and moving of videoconferencing or technical equipment for installation and troubleshooting is required. Occasionally climbs ladders to adjust overhead equipment and lifts or moves objects weighing up to forty pounds; occasionally lifts or moves heavier items with assistance from others.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
