Home Based Primary Care (HBPC) is a group effort between the Veteran, family, caregiver and VA. The program uses a multi-discipline team of health care professionals (nurses, doctors, social workers, dietitians, etc.) who work together to create a plan of care specific to each Veteran.

Responsibilities

The program is designed for homebound Veterans to:
  • Improve quality of life
  • Restore health so the veteran can function at the highest level
  • Help veterans achieve their greatest degree of independence
  • Reduce the need for admissions and re-admissions to the hospital
  • Reduce the length of hospital stays
  • Reduce the number of outpatient visits
  • Support caregivers in their desire to keep a loved one at home

    Home Based Primary Care Provides:
  • Medication management, teaching and how to administer the medication
  • Wound care
  • Pain management
  • Laboratory draws
  • Tele-home care
  • Care coordination between VA and community care providers
The duties and responsibilities include but are not limited to the following:

1. Serve as a Staff Physician

2. Assumes physician leadership role in answering and following all Home-Based Primary Care consults in a timely manner in conjunction with the Home-Based Primary Care team. Actively engages in case finding, developing and maintaining Home Based Primary Care program education for the facility, quality improvement and administrative functions of the Home-Based Primary Care program.

3. Acts as a liaison and primary care provider for the Home-Based Primary Care agencies.

4. Covers a cap limit of 75 patients enrolled in the Home-Based Primary Care program and participates in HBPC PI meetings as well as Geriatrics and Extended Care staff meetings.

5. Works with the Home-Based Primary Care team to develop optimal care for the veterans enrolled in the Home-Based Primary Care program.

6. On a consultative basis, Home Based Primary Care patients will receive care evaluation and follow up of care planning initiatives.

7. Assist with training students, residents and fellows in rotation for Geriatrics and Extended Care Service Line.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
  • Proficiency in spoken and written English.
  • Physical requirements outlined below.
Preferred Experience:
  • Board Eligible / Board Certified in Internal Medicine or Family Medicine is desired
  • Experience in geriatrics and longitudinal patient care is desired.

Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office.

Physical Requirements: Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required.Heavy lifting, 45 pounds and over; light carrying, under 15 pounds; reaching above shoulder; use of fingers; both hands required; walking (8 hours); standing (8 hours); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); mental and emotional stability; clear speech. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Rigid deadlines and need for high degree of accuracy periodically becomes stressful.

Annual leave shall accrue for full-time physicians at the rate of eight (8) hours per biweekly pay period.
